received May 21, 2020. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on August 31, 2020.

rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections 1 through 4, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with one

2. Site Map: Please provide an updated site map in accordance with 15A NCAC 02T .0105(d), per Form
NDSR 01-20 Section III.1.
3. Groundwater Monitoring Wells: Please complete the renewal application Form Section IV.
Groundwater Monitoring Wells and provide the coordinates of the groundwater monitoring wells.
4. Setback Waivers, Easements, and Leases: Setback waivers were specified in the permit dated
September 11, 2015, Condition IL16.a., footnote a. and b. Please provide copies of supporting
documents and complete the Section V of the application Form NDSR 01-20, per 15A NCAC 02T
.0706(e).
