This inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in Non -discharge Permit No. WQ0036766. There were no significant issues or findings noted during the inspection and therefore, a response to this inspection report is not required. The purpose of the evaluation was to verify that the facility is operating in compliance with the conditions and limitations specified in Permit WQ0036766. Lee Buck, ORC, was present during the inspection. This facility consists of dual train, below -grade Moving Bed Biofilm Reactor (MBBR) systems. It is permitted for 15,000 gpd but only averages 4,000 to 6,500 gpd from one large commercial building of medical and office spaces. This system has experienced operational issues due to low hydraulic and organic loading rates. A review of files, since the last inspection in September, 2018, shows five monthly exceedance violations for Total Nitrogen and two monthly exceedances for Total Phosphorous related to poor BNR performance. There have also been some exceedances of TSS. A review of groundwater monitoring reports indicate high TDS and Chloride in mostly MW #2, most likely due to natural occurrences. This system has three groundwater lowering wells and a condition in the permit that, in the event 10,000 gpd of groundwater is withdrawn, the Permitte has to register and report annually to the Central Coastal Plain Capacity Use Area (CCPCUA). To date, the groundwater lowering wells have never generated any flow. The facility is compliant with non -discharge permit WQ0036766. Due to issues with the maintenance of the plastic UV bulb sleeves and rainwater shorting out electrical system, chlorine tablets are used for supplemental disinfection. Comment: Both infiltration basins were free of effluent durina the insDection. Grass covers the entire bottom of basins. A reminder, as required by the permit, the basins shall be periodically cleaned to remove deposited materials that may impede the infiltraion process. There are three monitoring wells which appeared to be in good condition and locked. Environmental One samples and analyzes these wells three times a year. The sign restricting access to infiltration basins was missing and needs to be replaced. Page 7 of 7
