I was looking over the major modification information provided on May 26. Everything looks good except for a few
small issues:

Please correct the following:
- General:
o Please upload electronic copies of the submittal materials (Application, Supplement-EZ Form, O&M
Agreement Form, plan sheets, calculations, and other information) to Laserfiche
(https://edocs.deq.nc.gov/Forms/SW_Project_Submittal).
- Application:
o Section II, 1a – Please indicate that this submission is a modification, not a new submittal.
o Section II, 1b – Please provide the existing permit number (SW6191006) and issue date (2/3/2020).
o Section IV, 8 – The percent impervious area should be calculated as the total amount of proposed BUA
that will exist on-site, post-construction (83,250 sf) divided by the total project area (3.47 ac).
- Supplement-EZ Form:
o Drainage Areas Page:
 Drainage Area 2, Line 5 – The total BUA within drainage area 2 should be a summation of Lines
6, 7, 9, & 13.
 Drainage Area 3, Line 16 – The provided value (90%) does not match the provided calculations &
sheets (89%).

Please provide the following:
- 2x hard copies of the revised Application pages and 1x electronic copy of the full revised Application. NOTE: The

Application does not require a new signature. The April 28, 2020 one will suffice.
- 2x hard copies of the revised Supplement-EZ Form pages and 1x electronic copy of the full revised Supplement-

EZ Form. NOTE: The Supplement-EZ Form does not require a new signature. The June 4, 2020 one will suffice.
- 1x electronic copy of the revised O&M Agreement Form, plan sheets, calculations, and other submittal
information.

Since these are really minor things, once I get the hard copies, anything else that may have changed from addressing
these issues, and the documents are uploaded to Laserfiche (Provided everything is still in order), we can issue the
modification without doing another formal 30-45 day review.
