We would like to request a modification of this permit. Although 'the general scope of the project remains the same, we have realized that there are changes in the project plans and impacts which need to be correctly reflected in the permit, prior to the letting of the project. The scope of the project includes intersection improvements within the median in the vicinity of the intersection and the construction of two bulb-outs for cross-over turning movements. A permit modification is being requested for the following revisions: • Wetland impacts at Bulb-out 2 (Plan sheet 5 and associated permit drawing). It was discovered that the slope stake line was omitted from the original submittal and therefore impacts need to be adjusted accordingly to 0.06 acres. • Wetlands were not shown adjacent to Bulb-out 1 (Plan sheet 2). There will be no impacts to these wetlands. These two changes, are shown in the attached plans, permit drawing for Bulb-out 2, impact summary table, and are referenced in the attached revised PCN. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: In the general vicinity of the project limits there exists commercial properties along US 17, residential developments, and forested land. In the immediate vicinity of Bulb-out 2, there is a maintained powerline corridor and a mowed grassed parcel which is currently undeveloped. 31b. Explain the purpose of the proposed project: To improve safety at the intersection of US 17 and SR 1561 Sloop Point Rd. 3e. Describe the overall project in detail, including the type of equipment to be used: This project includes safety improvements to the intersection of US 17 and SR 1561 (Sloop Point Rd). The improvements include new concrete islands, median work to construct and modify turn lanes, and widening for bulb-outs to accommodate U-turn movements in two locations. All work is shown on the attached plans. The bulb-out adjacent to the S-bound lane (Bulb-out 2) is the only widening that will impact jurisdictional wetlands. Bulb-out 2 will be 225 feet in length and will require widening of 0-35 feet from the existing edge of pavement. There are wetlands present in the vicinity of the bulb-out adjacent to the N-bound lane (Bulb-out 1). However, the wetlands fall outside of the area which will be impacted by Bulb-out 1.
