The following are corrections that must be made regarding your NOI submission for Greenlea 68 Site:
• Ala—the project name must match what's on the Letter of Approval(LOA).
• A3—the street address must match what's on the LOA.
• A7,8—use the coordinates on the LOA.
• Al5a—the receiving waterbody name is simply `Reedy Fork.'
• A15b—the index number must be written exactly as it is on the NC Surface Water Classifications map.
• 136,7—missing a `1' in the address number.
• D8—correct the project name on the NOI Certification Form.
• E—Mr. Collett is the person named on the NOI, so select the first radial button.
• E—the organization listed must be the same one that's in B 1.
A new NOI submission is required. To pre-populate the new NOI, select'Yes' for the first question and enter NOI number
