My contractor provided me with the attached sketch showing the work zone. He said no vegetation 3" DBH or
greater will be removed.

from the additional information request emailed on August 12. The application states that the corridor will be no
greater than 10 feet wide and will have little to no impact on vegetation. Please clarify the following:
• Please respond with information as to the location and dimensions for your construction access corridor through
the 50 foot buffer to the work area.
• Please indicate where the corridor will be located on the sketch diagram, what the dimensions will be, and whether
any woody vegetation 3" DBH or greater are proposed for removal.
• If vegetation is proposed for removal, please show the location on the sketch diagram. Include the above
information if a construction corridor will be utilized at the shoreline.
• Any trees that are removed must be replaced.

Thanks, Amy. I didn't realize emailing it was an option. Please see attached. My contractor (cc'd here) said the
work would be performed at about 8' off the shoreline (from land).

Just a reminder that the deadline to submit additional information to DWR is 9/12/2020. The application will
be returned to you if the requested information is not received. If the application is returned, a new
application and fee will have to be submitted to DWR if you propose to stabilize the shoreline.

On June 24, 2020, the Division of Water Resources (Division) received your application requesting a 401
Water Quality Certification from the Division for the subject project, with the appropriate fee received on
July 2, 2020. The Division has determined that your application is incomplete and cannot be processed. The
application is on hold until the following items are addressed:
• The application you completed is an older version. Please use the attached applications in the future or
access the forms on our website here: https://deq.nc.gov/about/divisions/water-resources/water-quality-
permitting/401-buffer-permitting-branch/application
• Please provide a signed copy of the application. If the application is not signed by the owner/applicant,
please provide an agent authorization letter delegating signature authority. A sample agent authorization
letter is attached. [15A NCAC 02H .0502(f)]
• Please provide all requested information in the II. Project Information section of the application. This
includes items No. 1 and No. 3 in this section.
o No. 1 - Provide a plat map or street map. Include the approximate location and extent of the
proposed shoreline stabilization (this can be hand drawn).
o No. 3 Provide photograph(s) of the shoreline / buffer proposed to be stabilized. Include a
shovel or yard stick for scale and show eroding areas.
• Please provide information as to the location and dimensions for your construction access corridor
through the 50 foot buffer to the work area. Please indicate where the corridor will be located on the sketch
diagram, what the dimensions will be, and whether any woody vegetation 3" DBH or greater are proposed
for removal. If vegetation is proposed for removal, please show the location on the sketch diagram. Include
the above information if a construction corridor will be utilized at the shoreline.
• Please provide what the length of the proposed rip rap along the shoreline (in linear feet). Please also
provide the proposed impact dimensions for below and above the full pond level in the following format:
o Above full pond: x feet x 4 feet = x square feet
o Below full pond: x feet x 1.5 feet = x square feet
Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information
for the proper consideration of the application. If all of the requested information is not received by
September 12, 2020, the Division will be unable to approve the application and it will be returned.
