Attached is the Rescission Request Form to rescind permit NCG030317 for the Hayesville Plant
located at 788 Tusquittee Road, Hayesville, North 28904. Industrial activities ceased as of March 31,
2020, and all outside equipment and containers were removed prior to June 30, 2020.

section 3 of the Rescission Request Form.
