Enclosed is a copy of the Compliance Evaluation Inspection Report for the inspection conducted at the subject
facility on August 18, 2020, by Mr. Wes Bell of this Office. Please inform the facility's Operator -in -Responsible
Charge (ORC) of our findings by forwarding a copy of the enclosed report.
This report is being issued as a Notice of Deficiency (NOD) due to the following:
- One UV display box (shows intensity, hours, etc.) was not operational and the information on the second
UV display box could not be observed;
- The visual indicators (on/off status) were not operational on all UV lamps.
Please be advised that the failure to properly maintain the wastewater treatment facility is a violation of the
subject NPDES Permit and North Carolina General Statute (G.S.) 143-215.1 as detailed in the UV Disinfection
Section of the attached report. Please be advised that G.S. 143-215.6A provides for a civil penalty assessment of
not more than twenty-five thousand dollars ($25,000.00), or twenty-five thousand dollars ($25,000.00) per day
when the violation is of a continuing nature, against any person who violates or fails to act in accordance with the
terms, conditions, or requirements of any permit issued pursuant to G.S. 143-215.1. Penalties may also be assessed
for any damage to surface waters of the State that may result from the violations.
It is requested that a written response be submitted to this Office by September 15, 2020, detailing the actions
taken to address the maintenance issues noted on the UV disinfection system. In responding, please address your
comments to the attention of Mr. Wes Bell.

Comment: The records reviewed during the inspection were organized and well maintained. Discharge
Monitoring Reports (eDMRs) were reviewed for the period July 2019 through June 2020. All
monitoring frequencies were correct. A weekly average effluent fecal coliform violation was
reported in March 2020. The Division has separately addressed this limit violation through
the issuance of a Notice of Violation (NOV).
The December 2019 and February 2020 through June 2020 eDMRs will be revised and
resubmitted to remove the "0" ug/L values in the TRC column. The facility does not use
chlorine; therefore, no values should be placed in this column.

Comment: The subiect Dermit reauires influent BOD and TSS influent composite samples. The Town
was approved to collect time -based composite samples by the Division on 2/24/12
(Compliance Evaluation Inspection report date). Weekly sampler aliquot checks are
performed and documented. The aliquot volume was initially below 100 mis. during the
on -site inspection; however, the ORC corrected the sampler program to ensure a minimum
of 100 mils per aliquot will be collected during future sampling events. Note: The facility was
not sampling during the inspection.

Comment: The sludge blanket in the #2 train clarifier needed to be lowered and excessive floating solid
was observed in the #1 train clarifier. Although no excessive solids were observed going
over the clarifier weirs, the ORC and staff must ensure floating solids are kept at a minimum
and the sludge blankets are properly maintained.
