Modifications to the subject Attachment are as follows: Addition of a new residual source Lear Corporation WWTP (Permit No. NC0002305) increasing the approved residuals land application to a maximum of 2,675 dry tons per year. The modified Attachment A shall be effective from the date of certification until September 30, 2021 (i.e., expiration date of the current permit), shall void Attachment A of Permit No. WQ0034806 issued October 11, 2016 that was previously certified on June 26, 2020, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace Attachment A of Permit No. WQ0034806 that was certified on June 26, 2020 with the modified Attachment A. Please note that the Permittee shall submit a renewal application no later than April 3, 2021. If any parts, requirements or limitations contained in this certification are unacceptable, the Permittee has the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding. Monitoring frequencies are based on the actual dry tons applied per year using the table below, unless specified above.
