Please find attached a payment (Check No. 167686) in the amount of One Thousand One
Hundred Fifty -Two Dollars and Eight Cents ($1,152.08). The payment pays in full the three (3)
administrative penalties assessed against Polk County. The attached payment is to be allocated as
follows: a total of Four Hundred Fifty Dollars ($450.00) to the civil penalties; a total of Three
Hundred Fifty -One Dollars and Ninety -Two Cents ($351.92) to the investigative costs; a total of
Two Hundred Thirty Dollars ($230.00) to the legal costs; and a total of One Hundred Twenty
Dollars and Sixteen Cents ($120-16) to interest.
The attached chart lists all three (3) administrative penalties, from June 26, 2017 through
September 5, 2017, and details how the payment is to be allocated to each.
