Division of Water Resources' Central and Regional staff has reviewed the application package
received March 19, 2020. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on July 3, 2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A and B, or failure to provide the additional information on or before the above requested date
may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

A. Soil Evaluation:
1. There were no soil sample results submitted for YK-42-05. Sample ID's that were included indicate
1., 2., 3. and 4. Please submit the soil sample results as per 15A NCAC 02T .I 105(a) and (b).
2. The soil samples submitted for new fields were missing Mercury and Molybdenum. Please submit
the soil samples as per 15A NCAC 02T .I 105(a) and (b).
B. Site May:
1. Please re -submit the new field maps pursuant to Division's Map guidance policy and 15A NCAC
02T .I 104(c)
2. There are surface water features (ephemeral, intermittent and perennial streams) on and adjacent to
the land application areas not shown on the site map. Please re -submit the maps pursuant to 15A
NCAC 02T .1104 (c)(1)(13).
3. There are drainage features on and adjacent to the land application areas not shown on the site map.
Per 15A NCAC 02T .1104 (c)(1)(13) all drainage features within 500 ft. of the site must be shown
on the setback maps.
4. No property lines are shown beyond the exterior boundary of each site parcel. Per 15A NCAC 02T
.1104 (c)(1)(D) all property lines within 500 ft. of the site must be shown on the setback maps.
5. None of the fence lines, onsite buildings, barns, etc. are shown on the setback maps. Per 15A NCAC
02T .1104 (c)(1)(A) all structures, including fences which are located within the land application
area shall be shown on the setback maps.
6. Many road names have not been labeled on the setback maps. Per 15A NCAC 02T .1104 (c)(1)
setback maps to accurately depict the location, orientation and relationship of the land application
fields. This includes labeling all Road Names on the map.
7. As per the Division's Map policy private roads, driveways and farm access roads that run through
or adjacent to the land application sites should be labeled.
8. Field YK-42-04: The field has a drainage way with a channel present that is not shown on the
setback map (see attached map sketch). The site map needs to be revised to show this feature and
the applicable buffer. There is also a perennial stream that flows adjacent to the entire site needs to
be added to the site map.
9. Field YK-42-01: The map needs to be re -submitted to include the following:
• There is a stream which flows along the western border of the application area, below the
pond which is not shown on the setback map (see attached map sketch). The site map needs
to be revised to show this stream and the applicable surface water setback.
• There is an access road that runs east to west which is not shown on the setback map. This
access road has a ditch that runs adjacent to the north side for the entire stretch (see attached
map sketch). The site map needs to be revised to show the access road, ditch and applicable
ditch setback.
