Elm St. Graham, NC 27253- Subject: Alamance County Health Building Rudd Street Alamance Tax Map: 6-46-151 Permit Number: 664 Dear Mr. Buddy Whitesell: The Engineering Department has reviewed the sedimentation and erosion control plans received on 06/29/2020, and find these plans to be acceptable and hereby issue this "Letter of Approval." This plan approval shall expire no more than three (3) years following the date of approval, if no land -disturbing activity has been undertaken, as is required by Title 15A NCAC 4B .0129. Please note that this approval is based in part on the accuracy of the information provided in the Financial Responsibility Form, which you provided. You are requested to file an amended form if there is any change in the information included on the form. The City's Sedimentation and Erosion Control Ordinance is a performance -oriented program requiring protection of the natural resources and adjoining properties. If following commencement of this project it is determined that the plan needs to be modified, then this office will require that the plan be modified to comply with the City's ordinance. Please consider this letter the Notice required by Section 31.5-18 of our right of periodic inspection to insure compliance with the approved plan. Please be aware that effective April 1, 2019, all new construction activities are required to complete and submit an electronic Notice of Intent (NOI) form requesting a Certificate of Coverage (COC) under the NCGO10000 Construction Stormwater General Permit. This form MUST be submitted prior to the commencement of any land disturbing activity on the above named project. The NOI form may be accessed at deg.nc.gov/NCG01. Please direct questions about the NOI form to Annette Lucas at Annette.lucas(a-)ncdenr.gov or Paul Clark at Pau l.clark(a)ncdenr.gov. After you submit a complete and correct NOI Form, a COC will be emailed to you within three business days. A copy of the Fact Sheet on the New NCG01 Permit enclosed with this letter for your convenience. Title 15A NCAC 4B .0118(a) and the NCG01 permit require that the following documentation be kept on file at the job site: 1. The approved E&SC plan as well as any approved deviation. 2. Separate sites from which borrow material is obtained, and which is not regulated by the provisions of the Mining Act of 1971, shall be considered as part of the land -disturbing activity for this site. Separate sites to which waste material is disposed of, other than permitted landfills regulated by the NCDEQ's Division of Waste Management, shall also be considered as part of the land - disturbing activity for this site. These off site areas shall therefore be included and shown on the approved erosion control plan and have the necessary erosion control measures in place. Acceptance and approval of this plan is conditioned upon your compliance with Federal and State water quality laws, regulations, and rules. This could include the Division of Water Quality under storm water or water quality regulations; the U.S. Army Corps of Engineers under Article 404 jurisdiction; county, city or town agencies under other local ordinances; or other approvals that may be required. Please be aware that this approval does not supersede any other permit or approval. Check with the local North Carolina Department of Transportation office to determine if an encroachment will be needed for any operations on DOT right-of-way. Permits will be required for driveways. Please have your contractor notify the Engineering Department, phone number (336-222-5050), to schedule a preconstruction conference so that we may discuss the Erosion Control Plan prior to initiation of construction. Please be advised that the Engineering Department will request a copy of your COC and applicable NCDOT encroachments be submitted prior to the request for a preconstruction conference.
