EXPRESS REVIEW ACCEPTANCE LETTER

STORMWATER MANAGEMENT PLAN (SMP)
On February 16, 2010 the Division of Water Quality DWQ received a request from Mr. Michael Stocks
of Stocks Engineering, PA (through Mr. Jeff Harbour of ESI) for acceptance into the Express Review
Program regarding the Stormwater Management Plan (SMP) review for the subject project in Nash
County. This letter advises you that your project will be accepted into the Express Review Program once
the following items are received:
a. The application fee of $1500.00 made payable to the North Carolina Division of Water Quality
[Stormwater Management Plan (SMP) with three (3) drainage areas];
b. Three (3) complete and collated copies of the Express Review Stormwater Management Plan
Application [http://portal.ncdenr.or web/wq/swp/ws/401/express/fortnsl;
Three (3) collated copies of all supporting information pertaining to this project including
i. Design calculations for the proposed BMPs sized for both on-site and any off-site
drainage;
ii. Most current BMP Supplement completed for each BMP including Design Summary
and Required Items Check List along with all required items (available at
http://portal.ncdenr.org/web/wg/ws/su/bmp-manual ;
iii. Signed and notarized Operation & Maintenance Agreements for the BMPs, available at
the same website in c.ii above.
d. Three collated copies of full size plans (with detailed construction information) including, but not
limited to, the following:
i. Existing conditions map of project area;
ii. Proposed site grading plan showing existing and proposed contours, and delineated
drainage area to each proposed BMP;
iii. Erosion and sediment control plan;
iv. Detailed plan and cross sectional views of each BMP showing information such as
zones, pool levels, bottom and top elevations of berm, outlet structure-pipe system with
topographic contours to receiving stream (outfall), etc (at appropriate scales as listed in
the Required Items Checklist);

V. Construction sequence and planting schedule for the proposed BMPs;
e. One (1) data CD of full size plans in TIFF Group 4 format (black and white, not grayscale or
color). If the plans are too large to store in TIFF format, they can be stored in PDF.
Please note that a pre-application meeting is required prior to the submittal of your completed application.
The owner of the project must be present at the pre-application meeting. The clock for this project will
not start until receipt of the completed application package and required fee. If the drainage areas exceed
what is indicated on the Initial Interest Form and/or the services are different you may be required to
remit an additional fee.
