have judged it to be consistent with the proposed design.

statement, representation, or certification in any application package shall be guilty of a Class 2 misdemeanor, which may
include a fine not to exceed $10,000, as well as civil penalties up to $25,000 per violation.

Please find attached the following information for the requested extension permit:

The above information is provided as required for processing of the requested permit application for Northwood
MUPD — Phase 1 Infrastructure plans, consisting of eight (8) proposed commercial lots. This initial submittal is to
allow the project to install the necessary sanitary sewer infrastructure in order to subdivide and develop the
individual lots and therefore we are requesting that a Zero Flow Application be granted at this time. Each
individual lot submission (submitted as multiple lots at a time) will request a Daily Design Flow at the time of Site
Plan submission that will in-tum be a modification to this original permit. The estimated Daily Design Flow is
listed in the calculations as 120,000 GPD. The site will require a lift station to connect to the existing forcemain
on the east side of 15-501, the calculations for which are included in this submittal.
