I apologize I was not able to email you before the 315Y regarding the facility information for our Henrietta Quarry —
NCG020783. I had updated the outfall information in the website database, but inadvertently missed emailing you the
same information and one contact update. In looking at the site specific permit summary, I noticed it listed only 1
outfall as 100% stormwater. In the database I updated the information to reflect is there is one stormwater outfall
(SW01) and one process wastewater (001) at the site. Both discharge to Hog Pen Branch (Stream Index 9-41-23,
Waterbody Classification WS-IV).
For the permit contact information section, we would like to request myself and Van Medlock be added to this section. If
I need to provide more information, please let me know.
