(Former) 1300 Cunningham Avenue Winston-Salem, Forsyth County H&H Project No. LLF-005 Dear Madam/Sir: hart ''` hickman SMARTER ENVIRONMENTAL SOLUTIONS RECRVED MAY 2 9 2020 CEty 1 i-tAL. FILES DWR SECTION On behalf of LLFIex, LLC (LLFIex), Hart & Hickman, PC (H&H) is submitting the enclosed two (2) copies of the semi-annual stormwater Discharge Monitoring Report (DMR) for the site referenced above in accordance with the industrial stormwater permit (NCG030694). Please note there are metal exceedances in Stormwater Discharge Outfall (SDO) samples SDO-001 and SDO- 002 which is consistent with historical data indicative of background sources. Copper was detected above the benchmark of 0.010 mg/L for SDO-001 (0.013 mg/L). Zinc was detected above the benchmark of 0.126 mg/L for SDO-001 (0.274 mg/L) and SDO-002 (0.230 mg/L). In 2014, DEQ performed an inspection to assess compliance with the NPDES general stormwater permit to evaluate the drainage of the facility's five SDOs. The SDOs were found to be in excellent condition and concluded in a letter dated June 5, 2014 that offsite sources have opportunities to contaminate samples taken from outfalls 1, 2, 4 and 5. Considering the 2014 NC DEQ letter, there are no metal exceedances requiring a tiered response. Should you have any questions or need any additional information, please feel free to contact me. For purposes of this permit the definition of Total Toxic Organics is that definition contained in the EPA Effluent Guidelines for the facility subject to the requirement to sample (for metal finishing use the definition as found in 40 CFR 433.11; for semiconductor manufacture use the definition as found in 40 CFR 469.12; for electronic crystal manufacture use the definition as found in 40 CFR 469.22; and for cathode ray tube manufacture use the definition found in 40 CFR 469.31). The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement: "Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO), I certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or stormwater runoff has occurred since filing the last discharge monitoring report.
