I have been reviewing the permit for Frazier Pit Mine (NCG020952) and noticed some small issues: Question 19: Correct classification of waters should be "SA; HQW" Question 20: Should be marked "Yes, HQW*" Question 21: Should be marked "Yes, SA*" Question 38: Only parts a and b are filled out, parts c through f need to be. Can you please fill out another application with those issues addressed and send it in? It would greatly help me in processing this permit. Thank you so much!
