Dear Ms. Olson:
On August 24, 2020, we received a request to close the above referenced permit. The package included
historical analytical results, updated site map, and supporting letter from Ms. Julie Woosley, Section Chief
with the Hazardous Waste Section of the NC Division of Waste Management (DWM).
Information included in the package indicates sulfate will continue to be monitored in MW-5 by the DWM
until 02L standards are met, and wells associated with this permit will be closed per 15A NCAC 02C once the
remediation/monitoring is complete (copies of well abandonment records GW-30 forms will be provided to
the UIC Program). Based upon the stipulations included therein, this letter completes the monitoring and
compliance reporting requirements of your permit.
Any other compliance monitoring or reporting requirements of any other regulatory programs shall continue
as needed. If you wish to conduct future injection operations, you must first apply for and receive a new
injection well permit. Operating an injection well without a valid permit could result in a civil penalty of up to
$25,000 per day. If you have any questions regarding your permit or the Underground Injection Control
