Dear Mr. Rosenblatt,
Thank you for the concern you expressed during our phone conversation of last week regarding proper
reporting as per the terms of your NPDES permit. I also appreciate your faxing me the results of
sampling at The Hideaways WWTP during October and November.
As you sure ised, there is more that we ask of our permittees with regard to monitoring and reporting. I
have enclosed some copies of the form one should use when reporting characteristics of the effluent to the
Division. Looking at your permit, this is the only type of monitoring you are required to perform at this
facility. I've also attached some instructions as to how the forms are to be completed. You are requested
to transcribe the information from the laboratory reports onto the reporting forms for the months of
October and November 2005. Due to the circumstances surrounding your transition into ownership of the
property, you will not be able to provide all the information we would desire to see on a discharge
monitoring report (DMR) and what is required by your permit. I can only advise you to fill out the forms
as completely as. you possibly can, then certify the reports by affixing your signature on the back side of
the forms and send them to our Central Files office. It is hoped now that you have hired someone to serve
as your ORC, future reports will be submitted on time and with no deficiencies.
