This form must be completed by Permit Writers for all expedited permits which do not require
full Fact Sheets. Expedited permits are generally simple 100% domestics (e.g., schools, mobile
home parks, etc) that can be administratively renewed with minor changes, but can include
facilities with more complex issues (Special Conditions, 303(d) listed water, toxicity testing,
instream monitoring, compliance concerns).

Renewal Review and BIMS Downloads:
• Compliance history- one MV in 2015. No enforcements since 2010.
• TOX Testing Summary — 2016-2020 annual "pass" at 100% (see attach tox data
summary)
• DMR Parameter Values Export:
o Parameters not detected — BTEX, MTBE, Oil & Grease
o Total Recoverable Phenolics [code 32730] detected on two occasions at Outfall
001 (124 and 92 ug/L) and at Outfall 002 (41 and 120 ug/L)
o Permittee mistakenly entered "1" for all EPA method 625 tests. Permittee
misinterpreted "l=yes", thinking "yes" meant "test completed" rather than
"compounds detected." DMRs are being modified to correctly state no
detections.
RPA run on existing database — revision for final pending receipt of amended DMRs.
Limited dataset for phenols since only 2 detections per outfall. Continue Quarterly
monitoring.
Changes to previous version:
• Added HUC to outfall description on supplement to cover page.
• Updated parameter codes in section A(1) and (2).
• Updated eDMR reporting requirements section A (5) and toxicity reporting requirements
A (3) and (4).
• Removed footnote 5 which required toxicity tests to be collected concurrent with BTEX

Emily, as discussed the DMR reports from July 2016 to present have been revised to correct the
code number reported with semiannual SVOCs monitoring, changing it from 1 to 0(zero), as there
were no detections.
For the Charlotte 1 terminal DMRs from July 2016 to January 2020 were revised. The same revisions
were made for the Charlotte 2 terminal. Both were submitted and have been processed in eDMR.

wastewater discharge permit to the person(s) listed below. Written comments regarding the

This is in regards to the NPDES permit for Kinder Morgan Southeast Terminals LLC's Charlotte
Terminal 1/NC0032891.
In order to provide more convenience, control, and security to our permittees and assist them in
processing their transactions, The Division of Water Resources is currently transitioning towards
