Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:* The Bank is approx 4 feet high and we have lost approx. 3 to 4 feet of river bank in the last several years. This is the common area for the HOA for the 142 lot owners in the subdivision. There are existing access points to the river along the bank for tubing, kayaking, fishing etc. The river back is approx 500' long. 4b. Have Corps permits or DWR certifications been obtained for this project (including all prior phases) in the past?* r Yes r No r Unknown 4d. Attach an 8 1/2 X 11 excerpt from the most recent version of the USGS topographic map indicating the location of the project site. (for DWR) 4e. Attach an 8 1/2 X 11 excerpt from the most recent version of the published County NRCS Soil Survey map depicting the project site. (for DWR) 4f. List the total estimated acreage of all existing wetlands on the property: 4g. List the total estimated linear feet of all existing streams on the property: 4h. Explain the purpose of the proposed project:* We would like to slope the river bank back towards to the common area, because when the river rises over the bank, it drops sand at the back and cuts a bowl shape thru the middle of the common area. We would like to lower the bank down and use the soil that is there to fill in the bowl then place a mat down and roll the area to prevent further erosion of the bank. We have tried other methods of erosion control, silky dogwoods, etc, but eventually the river has eroded the bank so they ones that remain are no longer effective. This new project with take several years to complete as the HOA resources are limited. We would like to do approx. 75 to 100 feet per year until the bank is fully rocked over. Other private land owners have down this to their bank years ago and they Bank has held if soil and shape. 4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:* So the project would require the existing bank to be pulled back onto the common area and reseeded. We would need a grader to pull the bank back, then use a a construction mat before covering it over with large flat rocks. 4j. Please upload project drawings for the proposed project. 5. Jurisdictional Determinations 5a. Have the wetlands or streams been delineated on the property or proposed impact areas?* C Yes Comments: r No 5b. If the Corps made a jurisdictional determination, what type of determination was made?* C Preliminary C Approved C Not Verified r Unknown C WA Corps AID Number: 5c. If 5a is yes, who delineated the jurisdictional areas? Name (if known): Agency/Consultant Company: Other: 5d1. Jurisdictional determination upload 6. Future Project Plans 6a. Is this a phased project?* C Unknown r Yes C No 7b. If yes, explain. The HOA is a non profit and can not afford to complete the entire bank at once. We will use the same local company, Buena Vista Landscapes to do the work. We will try to complete approx. 75 to 100" per year. Are any other NWP(s), regional general permit(s), or individual permits(s) used, or intended to be used, to authorize any part of the proposed project or related activity? D. Proposed Impacts Inventory 1. Impacts Summary 1a. Where are the impacts associated with your project? (check all that apply): U r Wetlands r Streams -tributaries r Buffers [ Open Waters r Pond Construction E. Impact Justification and Mitigation U 1. Avoidance and Minimization 1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project: We have been planting silky dogwoods for the past 10 yearlong the bank, but the erosion continue. As stated earlier, the Bank is now higher then ever and it continues to increase each time to river breaks it back. 1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques: We will grade the back back, place the construction mat down and then police large flat rocks up the bank so the water will flow freely over the rock and continue down the river. 2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State 2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State? r Yes r No 2b. If this project DOES NOT require Compensatory Mitigation, explain why: F.
