BRIAN WRENN Director NORTH CAROLINA Environmental Quality August 25, 2020 CERTIFIED MAIL 7017 1450 00016450 5806 RETURN RECEIPT REQUESTED Town of Kernersville Attn: Wendi Hartup, CSM, Stormwater Manager 509 Michael St. Kernersville, NC 27284 Subject: MS4 PERMIT COMPLIANCE AUDIT NOTIFICATION Town of Kernersville NPDES MS4 Permit No. NCS000483 Forsyth County Dear Ms. Hartup: The Department of Environmental Quality (DEQ) staff will be performing a National Pollutant Discharge Elimination System (NPDES) Municipal Separate Storm Sewer System (MS4) Permit compliance audit for the subject permit. An on -site audit will be performed on September 23-24, 2020 beginning at 8:30 AM and is expected to take up to two (2) full days. Please arrange for an appropriate meeting room that will accommodate your representatives, 3-4 staff members from DEQ, and access to any electronic tools routinely utilized to implement the MS4 Program (e.g. databases, GIS, etc.) and notify me of the location. You are expected to ensure that all relevant staff, contractors and management/leadership/officials are present for scheduled audit meetings. On -site meeting rooms must provide adequate space for all participants to maintain appropriate social distancing protocols for COVID-19. If special arrangements or schedule adjustments are necessary to achieve this, please discuss this with me prior to the on -site meeting date. All participants of on -site meetings are expected to wear personal protective equipment in accordance with current state requirements, guidelines and recommendations; and to also maintain appropriate social distancing protocols for COVID-19 at all times. DEQ is also formally requesting submittal of required permit documentation prior to the audit. All submitted documentation will become a permanent public record of the compliance audit. Any documents that contain sensitive security information which should not be included in the public record must be denoted as such upon submission [Ref. N.C.G.S. 132-1.7]. Permit NCS000483 2. Stormwater Management Plan (SWMP) 3. Annual Reports not already submitted through DEQ's online BIMS SWMPA portal 4. MS4 Program organizational chart and associated program responsibilities for staff/positions S. MS4 map of outfalls and receiving waters 6. Formal stormwater program implementation agreements with any other entities 7. List of industrial facilities covered under NCS000483 8. Illicit Discharge Detection and Elimination Program Plan 9. MS4 Operation and Maintenance Plan 10. Municipal SCM Operation and Maintenance Plan 11. Post -construction ordinance reference or link.
