This letter is in response to the Notice of Violation, Tracking # NOV-2020-DV-0052 concerning the spill
that occurred on December 1 st, 2019 at 3906 Lasalle Drive, Guilford County, High Point, North Carolina.
Our plant maintenance staff arrived at the Lake Forest lift station at 5:03am after receiving alarms from
the lift station. They found water spraying from a pipe on to the electrical panel in the dry well. They
made repairs to the pipe and try to restart pumps. At 6:30am the primary pump had a mechanical issue
and shut itself down. A technician began repairing the pump when the backup pump failed. When the
backup pump failed a crew was in route with a bypass pump and piping. The bypass pump was
installed pumping operations began.
With both pumps down at the station and heavy rains the bypass pump couldn't keep up with flow
coming into the station causing the siphon box beside 3906 Lasalle Drive. to overflow. Both lift station
pumps and electrical controls were put back in service afterthe repairs where completed the following
morning. After the pumps were repaired and running, the overflow ceased, and the sewer lines were
pumped down to normal levels.
The spill was caused by an abnormal pump failure due to a flooded dry well.
