Division of Water Resources' Central and Regional staff has reviewed the application package
received February 10, 2020 and additional information received June 11, 2020. However, additional
information is required before the review may be completed. Please address the items on the attached pages
no later than the close of business on September 16, 2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items,
or failure to provide the additional information on or before the above requested date may result in your
application being returned as incomplete.

revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper

A. Facility Description:
The facility description for the existing 150,000 gallon per day (GPD) wastewater treatment system in the
last permit issued February 16, 2015 appears to be incorrect and needs to be corrected. There is confusion
with how many total units there are from the parallel treatment train described. Some units, like the
manually cleaned bar screen and two 25,000-gallon clarifiers, are listed twice in the description. Two
separate sets of units, the 37,500-gallon biological reactors and the 75,000-gallon aeration basins, both list
fine bubble aeration and it is believed that they refer to the same units. Please provide edits to the existing
facility description to accurately describe what is on site, and clarify how many of each treatment
component is present.
2. Old engineering plans and inspection reports suggest that the reclaimed water irrigation storage pond is
hydraulically connected to an adjacent golf course pond via wet well plumbing. Plans show that the pump
station at the irrigation pond is a transfer station that pumps to another existing irrigation system pump
system that also can pump from a freshwater pond. Any facilities that handle the treatment, storage or
distribution of reclaimed water must be accounted for in the permit, including any and all pump stations.
Please confirm what the reclaimed water irrigation setup is to get the reclaimed water from the effluent
storage pond to the approved fields, and provide pumping rates for any pump stations involved.
Review of previously Non -Discharge Application Reporting forms (FORM NDAR-1) detailing
application rates onto permitted fields suggest that irrigation has only ever occurred on Bay Course fields
(B-FW, B-GR, B-TEE, B-PER and B-RGH in Attachment B of the current permit). Has irrigation of
reclaimed water ever occurred on any field in the Sand Course (S-1 through S-9) or Piper Course (P-1
through P-9 and P-DR)? If not, are the facilities currently constructed to be able to irrigate on these
fields?
B. Reporting:
It was noted in an inspection report from 2006 that the field designations used at the time in the
NDAR-1 forms to report amount of wastewater applied to the approved Bay Course fields were
different than those that were listed in the permits. This was addressed in a letter sent December 8,
2009, and included in the January 21, 2010 permit. In that permit, Bay Course fields were changed
from being labeled by hole (B-1 through B-9), to being labelled based on five subzones: the greens,
shoulders, fairways, tees, and roughs. This segregation of the fields was done so that the field
designations in the NDAR-1 form would be the same as how irrigation was operated. This means
that there should be uniform application rates for each of the five subzones.
It was also noted that this segregation would also need to occur on the Sand and Piper courses, and
that irrigation of wastewater onto those courses be ceased until irrigation zones could be subdivided
similar to the Bay Course fields. However, this doesn't appear to have occurred yet. We need to
make sure that when irrigation is reported on the NDAR-1 for the fields in the Sand and Piper
courses matches what is listed in the permit. Please clarify how irrigation is reported for the fields
in the Sand Course (S-1 through S-9) and Piper Course (P-1 though P-9 and P-DR) currently.
C. Site Map:
The map provided in the additional information response package received June 11, 2020 did not include
any drawings for the fields in the Sand and Piper Courses. Please provide an updated site map for the
irrigation fields in the Sand and Piper Courses in accordance with 15A NCAC 02T .0105(d) that shows the
following:

2. Distance measurements verifying all setbacks are being met
3. Stormwater drainage controls
4. Location and ownership of property boundaries within 500 feet of the disposal area (including road/rail
right-of-ways)
5. All facility -related structures and fences within the treatment, storage, and disposal areas
6. All habitable residences and places of assembly within 500 feet of all treatment, storage, and disposal sites
7. Topographic contour intervals not exceeding 10 feet or 25 percent of total site relief
8. Soil mapping units on all disposal sites
9. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds, ditches, and
other surface drainage features within 500 feet of all waste treatment, storage, and disposal site(s)
10. Location and identification of each monitoring well (identify any background/upgradient wells)
11. Latitude and longitude coordinates of all monitoring wells (decimal degrees to 6 digits)
12. Location and identification of major components of the waste disposal system
13. Identification of all storage or treatment earthen structures that are part of the permit
14. The wetted perimeter of all irrigation/infiltration fields with field names (named according to the approved
permit)
15. The delineation of compliance and review boundaries
16. 100-year floodplain (if applicable)
