Based on the current guidance to minimize the spread of COVID-19, the Department of Environmental Quality has

It depends on what type of contact he is. If he is going to be an Owner/Legally Responsible person, choose the
211 form listed below. If he is only a Facility or Permit Contact, you may email me his information and who he
replaces.
I've run a Permit Summary Report, see attached, for your review.
Below is the portion of our website with links to the forms:
Manage Information for an Existing Industrial Permit
Enter your stormwater permit number into the Stormwater Permit Summary Report and you will see a form
that includes the address of the facility, contact information for the permit, and the regulated stormwater
outfall(s) for the facility.
To update the:
Facility name or ownership, complete the Name/Ownership Change Form.
Legally responsible person, complete the Owner Designation/Affiliation Form.
Facility address, email Suzanne McC
Billing contact, email Aziza Selkane.
• Stormwater outfall information, email Suzanne McCoy.
