Sciences Duke Energy Carolinas, LLC 52-6 South Church Street, Mail Code EC12J Charlotte, NC 28202 Subject: Additional Information Request UIC Permit Other Application No. W10300435 Duke AIlen Steam Station Plant Gaston County Sent electronically and via U.S. Postal Service Dear Mr. Wells: The Division of Water Resources (DWR) is in receipt of your application to construct and operate an Underground Injection Control (UIC) groundwater remediation system at the above referenced site. After review by staff, we find that additional information is needed so that we can continue the review and permit issuance process. Please address the following items within 30 days of receipt: 1. More detail is needed on how contaminates of interest (COls) and unrelated contaminants that may be present in surface water (e.g., PPAS/Pl~ OAs) will be monitored and reduced to prevent injection into the subsurface that may result in exceedances of groundwater standards per 15A NCAC 02L .0202. 2. More detail is needed in the Application on the Interim Monitoring Plan (RVIP) and the Environmental Monitoring PIan (EMT). This would include lists of parameters for both, time frame or schedule for transition from EYW to EMP, etc. 3. More detail is needed on how injection pressures will be managed to avoid creating preferential pathways. 4. More detail is needed on how backwash from the filter/UV system will be managed and disposed. 5. A number of injection and recovery wells are proposed to be installed in areas with potential heavy traffic. Will the well vault construction be sufficient to protect the well heads? Is there a plan to include reinforced concrete to the well --pad concrete to accommodate heavy equipment traffic? 6. More detail is needed in the timing of how and when injection wells will be installed in relation to removal of the coal ash pile_ During well installation and afterwards, the integrity of wells must be maintained (this includes the slanted wells). The Application does not clearly define where the surface water for the injection wells will be drawn from. Please submit a modified site map showing the location(s) of the fresh water intake. S. Please provide your phone number as this was not included on the UIC Permit Application. Please note that failure to provide this additional information on or before the above requested date may result in your application being returned as incomplete. If additional time is necessary, the Application may be withdrawn to avoid being returned as incomplete. Either the Applicant or the Applicant's consultant may request withdrawal by email message or letter if this is desired.
