Per our phone conversation yesterday the M-228 Exposure Repair (DWR # 20-0528) was completed Wednesday
afternoon and the site received approximately 3-inches of rain within an hour that evening.
Myself, Josh Pryzwara (Dominion Energy Environmental Compliance Coordinator) and Joey Lawler with S&ME visited the
site yesterday afternoon. We observed a turbid water release and sediment deposition downstream of the culvert and a
bank failure within the area of permit impacts.
Our contractor has already removed sediment downstream of the culvert and has installed wattles near the washout as
a temporary measure. Once the site dries out our contractor will bring in a mini excavator to fix the bank failure and will
stabilize the site.
Please find the attached pictures that were taken yesterday. Once the repair is complete we will provide you with
pictures and a five (5) letter will be submitted next week.
