the eNOI form. Please make sure to check the boxes when filling out the form. Additionally for Part A section 15b if you
use the NC DEQ water classifications map at

the address of your site you should be able to click on the portion of Hodges Mill Creek which will receive the drainage
from your site. Please make sure to enter to complete stream index including the final number found in parentheses in
your eNOI form. Lastly for Part A section 14 please include the estimated post-construction impervious surface area in
acres.
