On March 3, 2015, Mike Mickey of this office met with Otto Muha and Doug Witten to
perform a Compliance Evaluation Inspection on the stormwater management system at Motiva's
Greensboro Terminal. The facility was found to be compliant with the NPDES permit. The
inspection findings are detailed below and an inspection checklist is attached for your records.
I. Permit
The permit expires on August 31, 2016. Please note that at the next renewal the permit will require
that the monthly self -monitoring reports (DMRs) for Motiva be submitted electronically per new
EPA rules.
II. Self -Monitoring Program
Monthly self -monitoring reports (DMRs) were reviewed for the period January 2014 through
December 2014. The facility was found to be compliant with the NPDES effluent limits for the
above period. In regards to the permit monitoring requirements, it was noted that all sampling was
performed per the frequencies specified in the permit
III. Flow Measurement
The permit requires episodic flow monitoring. Flow is measured using a totalizing meter on the
effluent pipe. The discharge is controlled manually by a locked release valve. The flow totalizer
was replaced with a calibrated unit on December 1, 2014.
VI. Facilitv Site Review
The Motiva facilty has the following stormwater treatment structures:
• fueling island that drains to an open concrete holding pit with a sump pump.
• the pit water is pumped to a 8,000 gallon above ground tank with a hydrocarbon detector.
• diked secondary containment surrounding the hydrocarbon fuel and ethanol ASTs.
settling pond with locked effluent release valve and flow totalizer.

A review of the facility files showed that all records were being maintained according to the permit.
A comparison of the December 2014 DMR with the corresponding certified laboratory data sheets
from R&A Labs showed that all of the results were properly transcribed onto the self -monitoring
report. In addition, the retention pond draining report log properly documented the effluent release
dates and totalizer readings.
VI. Effluent/Receiving Waters
The system discharges to a rip rap lined ditch just outside of the perimeter fence. The effluent
eventually reaches a tributary of Long Branch, class "WS-IV " waters of the Cape Fear River
Basin. No effluent was being discharged on the day of the inspection.

The NPDES permit was reissued with an effective date of October 1, 2011. The permit expires next on
August 31, 2016.
II. Self -Monitoring Program

2013. The facility was found to be compliant with the final NPDES effluent limits for the above period.
In regards to the permit monitoring requirements, it was noted that all sampling was performed per the

pipe. The discharge is controlled manually by a locked release valve.
VI. Facility Site review
The stormwater treatment system consists of a holding tank for the fueling island, diked secondary
containment structure surrounding the above ground storage tanks, settling pond and a locked effluent
release valve. The facility appeared to be competently operated and maintained. It was noted that Motiva
is in the process of installing a 8,000 gallon tank that will follow the existing holding tank and will be
equipped with a hydrocarbon detector. This new tank will also discharge into the diked containment, area.

Comment: The Motiva stormwater treatment system was found to be complaint with

The stormwater management system consists, of the following:

site disposal).
* diked secondary containment stucture with settling pond.

A comparison of the Dec 2013 DMR with the corresponding operator's flow release
records and the laboratory data from R&A Labs showed that all values were transcribed
onto the monitoring report correctly.
Motiva is in the process of installing an 8,000 gallon tank that will follow the holding tank
at the fueling area. This tank will be equipped with a hydrocarbon detector to help
isolate spills. Like the existing holding tank, the new tank will also discharge into the
diked containment area.

declassify the subject permit and not require the designation of an ORC and Backup ORC.
This change in classification will be effective as of the date of this email and only applies to the classification and ORC
conditions. All other conditions and requirements of the permit are in full effect as issued.
I will make the necessary changes to the permit in our database.

Steve — Attached is letter from Motiva requesting that their certified operator requirement be eliminated (currently PC-
1).
Motiva's stormwater treatment system consists of a holding tank for the fueling island, a diked secondary containment
structure surrounding the above ground storage tanks, settling pond, locked effluent release valve and an in -line flow
totalizer.
Motiva simply manually releases stormwater that accumulates in the diked containment structure. There are no pumps
or motors. There is no chemical treatment and they do not have a groundwater remediation system (the permit
supplement page erroneously lists a GWR system).
I would have to agree that the PC-1 requirement does not seem applicable to their facility. How should we handle their

The Motiva Enterprises LLC (Motiva) Greensboro Terminal located at 101 S Chimney Rock Road,
Greensboro, Guilford County discharges stormwater from the facility under NPDES Permit No.
NC0022209. The Surface -Water Pollution Control System which handles stormwater in proximity to
above -ground storage tanks containing petroleum hydrocarbon fuels and ethanol at the terminal is
classified PC-1. Stormwater that collects on the property is conveyed to a settling pond and discharged
through a point -source outfall, Outfa11001, to an unnamed tributary of Long Branch stream through a
manually operated (normally closed) discharge control valve. Detention and settling are the only
stormwater treatment at the facility.
Motiva is requesting the Division of Water Quality reconsider the PC-1 classification of the terminal's
treatment system and eliminate the Certified Operator requirement. Since there is no "treatment plant"
at the facility and water samples are collected from a single discharge point, the requirement to
maintain two Certified Operators at the facility seems onerous. The facility is staffed with only three
employees so time spent away from the facility in annual continuing education classes impacts facility
operations. Furthermore, the operators have difficulty finding classes that relate to the type of system
they oversee as the approved classes typically cover more complicated treatment systems.
Motiva employs several pollution prevention Best Management Practices including spill prevention
and cleanup practices, material use controls, training and daily inspections. No discharge permit
excursions have occurred since the permit was issued in 2011. There is no groundwater remediation
system in operation and no plans or need to install groundwater treatment.

In addition, Motiva operates another terminal in North Carolina with a similar stormwater system
(settling pond with a single discharge point) and there is no Certified Operator requirement in that
facility's NPDES permit. I discussed this issue with Steve Reid, Extension Education and Training
Specialist, and he suggested I contact your office to request reconsideration of the permit classification.

On February 21, 2013, Mike Mickey of this office met with Doug Witten and Mike Mabarak to
perform a Compliance Evaluation Inspection at Motiva's Greensboro Terminal. This type of inspection
consists of two basic parts: an in office file review and an on -site inspection of the treatment facility. The
inspection found Motiva to be incomplete compliance with the NPDES permit. The inspection findings
are detailed below and an inspection checklist is attached for your records.
I. Permit
The NPDES permit was reissued with an effective date of October 1, 2011. The permit expires next on
August 31, 2016.
II. Self -Monitoring Program
Monthly self -monitoring reports (DMR's) were reviewed for the period January 2012 through December
2012. The facility was found to be in complete compliance with -final NPDES effluent limits for the above
period. In regards to the permit monitoring requirements, it was noted that all sampling was performed
per the frequencies specified in the permit. It should be noted that Part I.A.(2) of the permit requires that
the acute toxicity monitoring testing result be entered onto the effluent DMR in addition to completing
Form AT-1. Please make sure that the LC50 endpoint (>100, >90, etc) is listed on the DMR for the day
that the test is conducted.
Ill. Flow Measurement
The permit requires episodic flow monitoring. Flow is measured by a totalizing meter on the effluent
pipe. The discharge is controlled manually by a locked release valve.
VI. Facility Site Review
The Motiva facility has a diked secondary containment area for a fuel island and the above ground
storage tanks. Stormwater from the fueling area is routed to an evaporative tank. Plans are in the works
to replace this tank with an oil/water separator. Stormwater from the diked area is routed to a settling
pond with a locked effluent release valve. Although referenced on the Supplement to Permit Cover
Sheet, there is not a discharging groundwater remediation treatment system on site.

A. (3.) MTEE MONITORING - SPECIAL CONDITION
For the protection of public health, oil terminals discharging to waters classified as water
supply (WS) waters shall monitor Methyl Tertiary Butyl Ether (MTBE) for the duration of this
permit.
Outfall 001 If MTBE is detected at Outfall 001, the Permittee shall immediately resume
Monthly monitoring for MTBE, and the Permittee shall submit to the Division an MTBE Action
Plan to identify the sources of MTBE contamination. This Action Plan is due to the Division
within 180 days of the first detection of MTBE.
A. (4.) FLOC! MEASUREMENT
To determine the flow measurement, and calculate the runoff from a region, please utilize the
reference and information at http://portal.ncdenr.org/web/wq/ws/su/bmp-manual , given:
the runoff coefficient which accounts for infiltration and other
potential losses in the region,
the rainfall intensity to the region,
the time it takes for runoff to travel from the regions upper
reaches to its outlet, and the region's drainage area.

A. (2.) ACUTE TOXICITY MONITORING (ANNUAL)
The Permittee shall conduct annual toxicity tests using protocols defined as definitive in EPA
Document EPA/600/4-90/027 entitled "Methods for Measuring the Acute Toxicity of Effluents
to Freshwater and Marine Organisms." The monitoring shall be performed as a. Fathead
Minnow (Pimephales promelas) 24-hour static test. Effluent samples for self -monitoring
purposes must be obtained below all waste treatment. The permittee will conduct one test
annually, with the annual period beginning in January of the calendar year of the effective date
of the permit. The annual test requirement must be performed and reported by June 30. If no
discharge occurs by June 30, notification will be made to the Division by this date. Toxicity
testing will be performed on the next discharge event for the annual test requirement.
The parameter code for this test is TAE6C. All toxicity testing results required, as part of this
permit condition will be entered on the Effluent Discharge Form (MR-1) for the month in which

chlorine is employed for disinfection of the waste stream.
Should any test data from either these monitoring requirements or tests performed by the
North Carolina Division of Water Quality indicate potential impacts to the receiving stream, this
permit may be re -opened and modified to include alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as
minimum control organism survival and appropriate environmental controls, shall constitute an
invalid test and will require immediate follow-up testing to be completed no later than the last
day of the month following the month of the initial monitoring.

A Compliance Evaluation Inspection was performed on the Motiva Enterprises, LLC — Greensboro Terminal stormwater
treatment facility on November 15, 2008 by Corey Basinger of the Winston-Salem Regional Office. The inspection

I1. Records/Reports
A review of the laboratory reports and Discharge Monitoring Reports (DMRs) revealed no discrepancy between the
generated data and the data submitted to DWQ. A copy of the laboratory data is kept on file. All applicable data is
collected as per the NPDES permit requirements
III Facility Site Review
The permit states that the treatment works consist of loading rack drains, oil/water separator, waste holding tank (with
optional pump -out) for offsite disposal, gravity drains to dike containment areas, and discharge control valves to
discharge to an unnamed tributary to Long Branch. At the time of inspection, the facility was installing new ethanol
tanks.
'IV. Effluent & Receiving Waters
The Motiva Enterprises, LLC — Greensboro Terminal discharges to an unnamed tributary to Long Branch (Class WS-
IV waters in the Cape Fear River Basin) On the date of the inspection, the stream was free of solids, grease, and foam.
As result of the fact that runoff from the newly constructed Bryan Blvd has the potential to affect water quality at the
permitted outfall, sample collection should continue at the outfall pipe and not in the stream.
V. Flow Measurement
Flow is measured utilizing an inline water meter at the discharge control valve. Annual calibration was conducted and

VI. Self -Monitoring Program
A review of the discharge monitoring reports (DMRs) for the time period of July 2009 through July 2010 showed that
the Motiva Enterprises, LLC — Greensboro Terminal experienced no permit effluent limitation violations.
