General — When submitting this application, please use the following instructions as a checklist in order to
ensure all required items are submitted. Adherence to these instructions and checking the provided boxes will
help produce a quicker review time and reduce the amount of requested additional information.

Unless otherwise noted, the Applicant shall submit one original and one copy of the application and supporting
documentation to the appropriate Regional Office (see page 3).
A. Cove�f Letter:
® Include a brief project narrative describing the final design (i.e system and/or pump station to ultimately
serve 500 homes, but flow for only 100 homes being requested now). For modifications, clearly explain
the reason for the modification (i.e. adding another phase, changing line size, length, etc).
B. App,' ation Form (FTA 10-14)
Submit the completed and appropriately executed Fast -Track (FTA 10-14) Application. Any
unauthorized content changes to this form shall result in the application being returned. If necessary
for clarity or due to space restrictions, attachment to the application may be made, as long as the
attachments are numbered to correspond to the section and item to which they refer. You do not need
t bmit detailed plans and specifications unless you respond NO to item B 13 .

The Applicant's Certification of the application shall be signed in accordance with 15A NCAC 02T
.0106(b). Per 15A NCAC 02T .0106(c), an alternate person may be designated as the signing official if
a delegation letter is provided from a person who meets the criteria in 15A NCAC 02T .0106(b).
C. Appl' ation Fee:

D. Certificate of Public Convenience and Necessity (For Privately -Owned Public Utilities Only):

Necessity from the North Carolina Utilities Commission demonstrating the Applicant is authorized to
hold the utility franchise for the area to be served by the sewer extension, or
❑ Provide two copies of a letter from the North Carolina Utilij( Comr sion's Water and Sewer )ivision
Public staff stating an application for a franchise has been received and that the service area is
contiguous to an existing franchised area or that franchise approval is expected.

E. Operational Agreements (For Home/Property Owners' Associations and Developers of lots to be sold):

F. Downstream Sewer, WWTF Capacity and Flow Tracking/Acceptance Form (-T'Sf 08-13)
❑ Submit the completed and appropriately executed Flow Tracking/Acceptance for Sewer Extension
Permit (FTSE 08-13) Form for all applications.
The applicant (and owners of downstream sewers, pump stations and/or treatment facilities submitting
form FTSE 08-13 certifies that the addition of the volume of wastewater to be permitted in this project
has been evaluated along the route to the receiving treatment plant, and that the flow from this project
will not cause capacity related sanitary sewer overflows or overburden any downstream pump station
en route to the receiving wastewater treatment plant.
Where the applicant is not the owner of the downstream sewer, submit two copies of form FTSE 08-13
from the owner of the downstream sewer and owner of the WWTF, if different.
The flow acceptance indicated in form FTSE 08-13 must not expire prior to permit issuance and must
be dated less than one year prior to the application date.
Submittal of this application and form FTSE 08-13 indicates that owner has adequate capacity
and will not violate G.S. 143-215.67(a).
Intergovernmental agreements or other contracts will not be accepted in lieu of a project -
specific FTSE 08-13.
G. Site Map (All Application Packages):
Submit an 8.5-inch x 11-inch color copy of a USGS Topographic Map of sufficient scale to identify the
entire project area and closest surface waters. Each map must include at a minimum:
➢ The location of the sewer line and pump stations and be of reproducible quality.
➢ Downstream connection points and the permit number for the receiving sewer (if known)
➢ Pump Station Locations and the longitude and latitude for each pump station (if applicable)
❑ Include a street level map showing all relevant project areas.
H. Str n Classification (WSCAS 01-15)
9" Submit the completed and appropriately executed Watershed Classification Attachment form
(WSCAS 01-15) if any portion of the sewer system project is within 100 feet of any surface water or
wetlands.
A variance must be requested for encroachment within required setbacks or buffers pursuant to
NCAC 02T .0305(f)
Environmental Assessments (Projects subject to an Environmental Assessment (EA)):
Projects involving an Environmental Assessment per 15A NCAC 01 C .0408, must be submitted for a
full technical review and must be submitted to the P_ ER_ CS Unit on application forms provided by the
Division.
Alternative Sewer Systems
Projects involving low pressure sewer systems, vacuum sewer systems and other alternative sewer
systems must be submitted for a full technical review and must be submitted to the PERCS Unit on
application forms provided by the Division.
K. Flow Direction
Many wastewater treatment systems are entering into agreements for regionalization efforts and
emergency treatment capacity. Parts of the system are installed so that the wastewater flow can be
directed to more than one treatment facility. If this is the case with the project, please indicate in B(12)
and give the permit number of the second facility.
L. Certifications — Section C
The application must be certified by both the applicant and the design engineer who is a North Carolina
Registered Professional Engineer (PE). The applicant signature must match the signing official listed in
Item A(1 b). The PE should NOT certify the application if he/she is unfamiliar with 5A NCAC Chapter
2T, The Gravity Sewer Minimum Design Criteria (most recent version) and the Minimum Design Criteria
for the Permitting of Pump Stations and Force Mains (most recent version), as applicable to the project.

Along with this form, submit a color copy of a USGS Topographic Map to identify the project area and
waterbodies. Each map or maps must show the location of the sewer system and include location identifiers
where the system traverses over or near waterbodies. The map should have location ID's for each different
waterbody and corresponding classifications should be recorded.

Enclosed per our conversation is the Fast -Track Application (FTA-10-14) package for the City of
Rockingham School Pump Station Replacement project. This project will consist of the construction of a
new 120 gpm duplex submersible pump station, 275 1f 8" gravity sewer to direct flow to the new pump
station, and 8611f of new 4" force main. The existing pump station is to be demolished and removed by
the City upon commissioning of the new station, and the old force main and approximately no if of
existing 8" gravity sewer are to be plugged and abandoned. No new sewer flows will result from this
project. The new pump station will have comparable horsepower to the existing station, but a higher
pumping rate, due to the characteristics of the selected pump and changes in head afforded by the new
force main alignment. The new force main will convey the wastewater to a separate tributary gravity
sewer serving the same downstream receiving pump station as the existing School pump station.
Therefore, there will be no net change in hydraulic loading of the downstream sewer system as a result of
this pump station replacement.
Additionally, we request approval of an alternate method of pressure testing the new force main. Per the
discussion you and had on February 24f, and as noted in the design calculations, the pumps will have an
operational TDH of approximately 27.5 ft. Line pressures, therefore will be low and likely never exceed 25
psi. The specified pipe material is Class 35o DIP and AWWA C9oo PVC, DR 18, which have pressure
rating of 350 psi and 165 psi, respectively. Given the low operational pressures, we propose to test the
force main at a hydrostatic pressure of too psi for a 2-hour duration. This is a variation from the
requirement given in DENR PS -FM MDC 03/o8, paragraph 6.04.C.3.c, which stipulates a test pressure of
1.5 times the maximum design pressure of the force main pipe material. Section 02494 of the Project
Specifications, included herein, further details the proposed testing requirements. Please confirm that
the proposed pressure test procedure will be acceptable to the Department.

Design Narrative:
Rockingham School Pump Station Replacement
February 2016
The City of Rockingham owns and operates an existing pump station located adjacent to Hwy 74 Business
on the west side of Hitchcock Creek and the southwestern side of the City. The existing station, identified
as the School Pump Station, serves an elementary school and several commercial establishments and,
historically, has had relatively low flows. It is a Smith -Loveless wet-pit/dry-pit type that has been in
operation since 1971 and is approaching the end of its service life. It is a too gpm station utilizing a 4"
force main to convey wastewater to a receiving manhole located on the east side of Hitchcock Creek,
which, in turn, is situated on a gravity sewer line that flows to the City's primary influent pump station to
its WWTP.
The service area of the pump station is, mostly, built -out and has little potential for further development.
Due to topography in the service area, the pump station flows cannot be re -directed via gravity sewer and
the City must, therefore, replace the existing station with a new one of comparable capacity. With low
potential for future growth, the new station will be designed to meet existing conditions and provide a
nominal increase in capacity, but no new flows will be produced as a result of, or permitted for, this
project. The new station will be a duplex, submersible facility utilizing non -clog pumps with above-
ground electrical panels and a separate valve vault. The selected pumps are to be capable of passing 3"
spherical solids, will have 4" discharge, and will mate to 4" station piping and 4" force main. The City has
acquired sufficient property to locate the new pump station approximately 250 if away from the existing
station in an area that is further away from the Hwy 74 right-of-way and Hitchcock Creek. New 8" gravity
sewer will direct flows into the new pump station. Additionally, the newly acquired property allows for a
new force main alignment that abandons the existing force main entirely and conveys the wastewater to a
separate gravity sewer line that flows to the same downstream pump station as the existing station. Thus,
the new School Pump Station flows are conveyed to the same point in the City's sewer collection system as
the existing, but follow a different route that doesn't utilize a bridge crossing or cross Hitchcock Creek.
The new School Pump Station will utilize 3 hp pumps and have a design flow of 12o gpm at 27.5' TDH.

According to City records, the existing station has average pump run times of 3 hours per week at too
gpm, or 2,500 gpd. Due to the nature of the flows being produced by the school and businesses, most of it
occurs during daytime and business hours, generally 6:0o am to 6:0o pm. Accordingly, avg influent
flowrates during these intervals are about 3.5 gpm. Allowing a peak factor of 2.5 produces a peak inflow
rate of 9 gpm. For design purposes, a io gpm influent rate is used to determine required wetwell working
volume at the design pumping rate. For the selected pumps, a standard 6 ft diameter precast concrete
wetwell is utilized. For this diameter wetwell, design for 2 vertical ft storage (working volume) and allow 1
of freeboard to influent pipe. Initial settings will allow for minimum 1 start per hour with pump run time
of approx. 3.8 minutes. At avg inflow rate, up to 6 hours of storage is available in the event of a power
failure. A manual transfer switch and generator connection is to be provided for alternate power supply.
In order to mitigate against flotation, the wetwell will utilize an 8" extended base. Flotation calculations
illustrate a net negative buoyancy for the wetwell as designed with groundwater elevation equal to the top
of the wetwell, or a fully submerged condition. The wetwell will be located in a flood zone and is to have
the top of the structure at the same elevation as the too-yr flood. In order to comply with DENR PS -FM
MDC 03/08, water tight covers will be provided for the wetwell and valve vault. Additionally, the wetwell
vent and the electrical panels are minimum 2 ft above the too -yr flood elevation.
