Inspection Summary:
Requested records were available upon request. It is recommended that the permittee develop a system to document and
track maintenance and line cleaning that is easily reviewable by town staff to ensure compliance with permit CIP was
available but should be updated to include current Capital Improvement needs. System map was available. A schedule
should be developed to test the portable back-up generator under operating conditions and document this testing. Grease
education materials are mailed in January and they are available on the town website. The spill response plan should be
updated to include current personel and contact information. Pump stations are visited daily. The main pump station has
been on diesel bypass pump for several years. The town's bypass pump recently went down and they were operating on a
rental diesel bypass pump. There was no SCADA, audible or visual alarms present at this station. Operator stated that the
town's pump had the alarms but the rental bypass pump did not. Functional audible and visual alarms are a permit
requirement. Immediate action should be taken by permittee to establish audible and visual alarms at this station. The lack
of SCADA or simple telemetry at this station makes this an even more urgent problem that needs to be addressed.

Comment: Schedule should be established to test portable back-up generator and document these tests.
Main pump station has been on a diesel bypass pump for an extended period of time. Town's
bypass pump went down and a rental unit is in place. There was no SCADA, audible, or visual
alarms functional at this station. Operator stated that alarms were present with the bypass
pump that is in for repairs but not on the currently in use rental bypass pump. Functioinal alarms
are a permit requirement. All other pump stations were functional and appeared to be well
maintained.
