In accordance with the application for a permit modification received on January 18, 1996, the Division is
forwarding herewith the subject NPDES permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated December 6, 1983.
If any parts, measurement frequ6cies or sampling requirements contained in this permit are unacceptable
to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following
receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150E of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer
27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and
binding.
Please take notice this permit is not transferable. Part II, EA. addresses the requirements to be followed
in case of change in ownership or control of this discharge.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.

to receiving waters designated as an unnamed tributary to East Fork Deep River in the Cape Fear
River Basin
in accordance with effluent limitations, monitoring requirements, and other conditions set forth in
Parts I, II, I1I and IV hereof.
This permit shall become effective July 1, 1996.
This permit and authorization to discharge shall expire at midnight on March 31, 2001.

SUPPLEMENT TO PERMIT COVER SHEET
Colonial Pipeline Company
is hereby authorized to:
1. Continue to operate six (6) oil/water separators discharging stormwater through Outfalls 001 -
005, and Outfall 006 for discharge of wastewater generated by hydrostatic testing of tanks
associated with facilities located at Greensboro Junction Tank Farm, 411 Gallimore Dairy
Road, Greensboro, Guilford County (See Part III of this Permit), and
2. Discharge from said treatment works at the location specified on the attached map into an
unnamed tributary to East Fork Deep River which is classified Class WS-IV waters in the Cape
Fear River Basin.

During the period beginning on the effective date of the permit and lasting until expiration, the Permittee is authorized to discharge
stormwater from outfall(s) serial number 001, 002, 003, 004, 005 and hydrostatic test water from outfall 006. Such discharges shall be
limited and monitored by the Permittee as specified below:

t Where no measurable discharge occurs, "no discharge" should be clearly noted on the submitted monthly discharge monitoring report.
Flow may be monitored in one of four ways: a) Measure flow continuously, b) Calculate flow based on the area draining to the outfall, the built upon
area, and the total rainfall (this method of flow measurement should not be used at facilities that have large ponds to collect
surface water runoff), c) Estimate by flow measurement at 20 minute intervals during the entire discharge event, or d) Base flow on pump logs.
Z Where possible, the grab sample for oil and grease should be sldmmed from the surface of a quiescent (calm water) zone.

There shall be no discharge of floating solids or visible foam in other than trace amounts.
There shall be no direct discharge of tank solids, tank bottom water, or the rag layer.
There shall be no direct discharge of tank contents following hydrostatic testing unless benzene concentration is less than 1.19 ug/1 and toluene
concentration is less than 11 ug/l.

E. ACUTE TOXICITY MONITORING (ANNUAL)
The peanittee shall conduct annual toxicity tests using protocols defined as definitive in E.P.A.
Document EPA/600/4-90/027 entitled "Methods for Measuring the Acute Toxicity of Effluents to
Freshwater and Marine Organisms." The monitoring shall be performed as a Fathead Minnow
(Pimephales promelas) 24 hour static test. Effluent samples for self -monitoring purposes must be
obtained below all waste treatment. The permittee will conduct one test annually, with the annual
period beginning in January of the calendar year of the effective date of the permit. The annual test
requirement must be performed and reported by June 30. If no discharge occurs by June 30,
notification will be made to the Division by this date. Toxicity testing will be performed on the next
discharge event for the annual test requirement.
The parameter code for this testis TAE6C. All toxicity testing results required as part of this permit
condition will be entered on the Effluent Discharge Form (MR-1) for the month in which it was
performed, using the appropriate parameter code. Additionally, DEM Form AT-1 (original) is to be
sent to the following address:

Test data shall be complete and accurate and include all supporting chemical/physical measurements
performed in association with the toxicity tests, as well as all dose/response data. Total residual
chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for
disinfection of the waste stream.
Should any test data from either these monitoring requirements or tests performed by the North
Carolina Division of Environmental Management indicate potential impacts to the receiving stream,
this permit may be re -opened and modified to include alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum
control organism survival and appropriate environmental controls, shall constitute an invalid test.
Failure to submit suitable test results will constitute noncompliance with monitoring requirements.

Discharge 004 consist of an O/W separator treating

and water from the wash down slabs in the meter out
S area. A second oil/water separator treats storm water
collected in the manifold yard subsurface drainage
system in the gasoline tank area. An underground
(pt��aCt spring also flows in the gasoline tank area.

Spray Irrigation: No available space due to tank bottom
water irrigation. Volume of stormwaters are too large.

for spray irrigation of wastewaters. Zone testing should be
analyzed on each hydrostatic test prior to discharge of tank
testing waters.
PART IV - EVALUATION AND RECOMMENDATIONS
The company has indicated that they will be making every
effort to further reduce erosion and minimize turbidity
through the utilization of mini -wetlands and vegetative
buffers. These activities may occur within the next few
months and should not be considered as part of the NPDES
permit requirements at this time. Should these activities
improve water quality conditions at this facility, the
Division should consider possible incorporation of similar
buffers at other diked terminal facilities. WSRO recommends
that the permit be issued with standard conditions for oil
terminal operations. The Water Supply Section, the City of
High Point and Guilford County Environmental Health (Ken
Carter) should also be ven the opportunity to comment on
