I spoke with Ms. Scott and she confirmed that the ORC signature had to appear on the mailed copy in addition to the
Approver (Operations Manager). She recommended the following process:
1. ORC Validates and Certifies EDMR, which goes to Operations Manager for Approval and Submittal.
2. 24 hours for processing. After the 24 hours,
3. Operations Manager should sign and send documentation back to ORC, who signs and submits to State of NC.
4. Both certifier (ORC) and approver (Operations Mgr.) should sign the EDMR.
I advised Ms. Scott that this is backwards from the Chain -of -Command and that this was also different from what was
requested by NC, since the inception of the EDMR's, which we were one of the first to perform.

On March 6, 2015, Mike Mickey of this office met with Barry Furr. . and Johnny -Tapia to
perform a Compliance Evaluation Inspection on Kinder Morgan's Greensboro Terminal #2 located
at 6376 Burnt Poplar Road. The facility was found to- be compliant with the NPDES permit. The
inspection findings are detailed below and an inspection checklist is attached.
I. Permit
The permit expires next on August 31, 2016. A renewal notice should be mailed to you from our
Raleigh office approximately 8 months prior to expiration.
H. Self -Monitoring Program
The monthly self -monitoring reports (DMRs) were reviewed for the period January 2014 through
December 2014. The facilitywwas found to,be compliant with final NPDES effluent limits for the
above period. In regarda to the permit monitoring requirements, it was noted that all sampling was,
performed per the frequencies specified in the permit.
III. Facility Site Review
There are two separate discharges of stormwater from the facility. The first involves stormwater
from the truck loading -rack drain area. The system consists of the following:
* oil/water separator
* 10,000 gallon below ground tank with pump
*two above ground storage tanks in series
*two carbon filtration units and a totalizing flow. meter
The second stormwater release is from the AST containment structure and consists of the following:
*diked containment surrounding the fuel AST's (no settling pond)
*manually controlled locked value for stormwater release from the dike area.

IV. Flow Measurement
The permit requires episodic flow monitoring. The discharge from the oil/water separator and
activated carbon system is measured using a totalizing meter. The discharge' from the diked
secondary containment structure is estimated using the rational method. Both discharges tie into the
same outall pipe.
V. Records/Reports
A comparison of the December 2014 DMR with the corresponding certified laboratory data sheets
from R&A Labs showed that all of the results were properly transcribed onto the. self -monitoring
report. Additionally, it was noted that the releases from the oil/water separator and the secondary
containment area are now accurately totaled on the DMR for the specific days that a discharge
occurs.
VI. Effluent/Receiving Waters
The discharge pipe is located outside the perimeter fence and empties to a roadside ditch along
Burnt Poplar Road. The effluent eventually reaches a tributary of East Fork Deep River, class
"WS-IV *" waters of the Cape Fear River Basin.. No effluent was being discharged on the day of
the inspection.

I understand. I've always been knowledgeable of the fact that "any" testing must be reported on the EDMR. I should
have looked closer at the test report to resolve the location discrepancy while you were still here. I've already
communicated with Sid Champion (R & A Labs) on the GN2 discharge information and he said that should not be a
problem.
My management was inquiring on a report and I told them that you normally get that out as soon as you can, most of
the time within a week or two.
I might have to discharge soon after the "thaw" if we get what they say we're supposed to get. Thanks for your
guidance.

Mike: Regarding the GN1 (NC0000795) inspection -Upon closer scrutiny on the December 2013 NPDES discharge test
results, I identified that the test results were for GN2 (0042501), NOT GN1. See the attached results, both for what you
observed yesterday, and the actual GN1 test results.
GN14th quarter MTBE test results were reported in October (15Y month of the quarter) as required. The December GN1
EDMR is correct with the indication of no MTBE testing for December 2013.
I apologize for the confusion.

flow meter. Stormwater from the secondary containment dike that surrounds the AST's is discharged
manually through a locked release valve. There is no settling pond in the diked area. The facility
appeared to be competently operated and maintained.

On February 27, 2013, Mike Mickey. of this office met with Jeff Neal and Cliff McCowan to perform
a Compliance Evaluation Inspection of Kinder Morgan's Greensboro Terminal #2 located at 6376 Burnt
Poplar Road. This type of inspection consists of two basic parts: an in office file review and an on -site
inspection of the facility. The inspection found Kinder Morgan to be in complete compliance with the
NPDES permit.. The inspection findings "are detailed below and an inspection checklist is attached for
your records.
I. Permit
The NPDES permit was reissued with an effective date of October 1, 2011. The permit expires next on
August.31, 2016.
II. Self -Monitoring Program
Monthly self -monitoring reports (DMR's) were reviewed for the period January 2012 through December
2012. The facility was found to be in complete compliance with final NPDES effluent limits for the above
period. In regards to the permit monitoring requirements, it. was noted that all sampling was performed
per the frequencies specified in the permit.
III. Flow Measurement
The permit requires episodic flow monitoring. Flow is calculated based on total rainfall per area. The
discharge is controlled manually by a locked release valve.
VI. Facility Site Review
The Kinder Morgan Terminal #2 facility has truck loading -rack drains followed by an oil/water separator
as well as diked secondary containment areas for the tank farm. This terminal does not have a settling
pond prior to discharge from the- tank farm. Although referenced on the Supplement to Permit Cover
Sheet, there is not an active discharging groundwater remediation treatment system.

A review of the facility files showed that all records were being maintained according to the permit. R&A
Labs is responsible for computing the daily flow values reported on the DMR's for the corresponding
release dates.
VI. EffluenVReceiving Waters
The system discharges to a the roadside ditch along Burnt Poplar Road. The discharge eventually
reaches a tributary of East Fork Deep River, class VS -IV *" waters of the Cape Fear River Basin.
Effluent was being discharged on the day of the inspection.

A Compliance Evaluation Inspection was performed on Kinder Morgan Southeast Terminals, LLC
wastewater treatment facility on October 13, 2010 by Corey Basinger of the Winston-Salem Regional
Office. The inspection consisted of two parts: an on -site inspection of the treatment facility and an in -
:office file review. The following are the findings from the subject inspection.
I. Permit
The NPDES permit for Kinder -Morgan Southeast Terminals, LLC became effective on March 1, 2007
and expires on August 31, 2011.
Il. Records/Reports
A review of the laboratory reports and Discharge Monitoring Reports (DMRs) revealed no discrepancy
between the generated data and the data submitted to DWQ. A copy of the laboratory data is kept on
file. All applicable data is collected as per the NPDES permit requirements.
III. Facility Site Review
The permit states that the treatment works consist of truck rack drains, an upper secondary containment
field for surface bulls -storage tanks, holding tanks (for excess upper diked area runoff), oil/water
separator, discharge control valves and discharge to an unnamed tributary to the East Fork Deep River.
IV. Effluent & Receiving Waters
The Kinder Morgan Southeast Terminals, LLC discharges to an unnamed tributary to East Fork Deep
River (Class WS-IV waters in the Cape Fear River Basin). On the date of the inspection, the stream .
was free of solids, grease, and foam.
V. Flow Measurement
Flow is measured as per permit requirements.

VI. Self -Monitoring Program
A review of the discharge monitoring reports (DMRs) for the time period of August 2009 through July
2010 showed that the Kinder Morgan Southeast Terminals, LLC experienced no permit effluent
limitation violations.
VII. Operations and Maintenance
No problems noted with respect to operation. Maintenance is performed as needed.
The previous inspection was conducted on October 13, 2008 by Corey Basinger of this Office.

Barry - The bypass of the gravity discharge structure is allowed under Part II. Section C. 4(c)(1)(A) of the NPDES permit which states: "Bypass was unavoidable to
prevent.... severe property damage". Mike.

Dear Mr. Sullivan:
The Division of Water Quality (the Division) received your site map and letter of July 30 stating your
intent to store ethanol in bulk at the subject facility. A permit modification is not required.
We understand that you intend to store this new fuel product in existing Tank #9 and monitor any
potential environmental impact utilizing your existing Water Pollution Prevention Plan under the
subject NPDES permit. The Division concurs with your assessment that changing the contents from
petroleum to ethanol in Tank #9, as monitored through permitted Outfall 001, does not require a
formal modification of your existing permit.
