This Permit shall be effective from the date of issuance until September 30, 1987, shall void permit 1477 issued August 21, 1968, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing before a hearing officer upon written demand to the Director within thirty (30) days following receipt of this permit, identifying the specific issues to be con- tended. sinless such demand is made, this permit shall be final and binding. Ramsey Winston-Salem Regional Supervisor Groundwater Section Pollution Prevention Pays RPW: jmp P.O Box 27687, Paler, North Carolina 2761IJ6+17 Tclephore 919-733-7-V5 An :_a.'31 ()7ror•Ur;?-v AfRrr. anvr 4v;;- r�..., NORTH CAROLINA • ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the Discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules and Regulations PERMISSION IS HEREBY GRANTED TO Lucks Incorporated Randolph County FOR THE continued operation of a 0.3 MGD non -discharge wastewater treatment and disposal facility to treat the combined sewage and industrial wastes of • the Luck's, Inc., Seagrove Plant, consisting of aerobic digestion and chlorination facilities for domestic waste pretreatment; washwater screening, grease removal and aerobic digestion facilities for process waste pretreatment; a 350,000 gallon capacity holding pond to collect the combined waste flow; and an automated spray irrigation system utilizing approximately 40 acres of woodland and grassland for the disposal of the treated waste- water with no discharge to the surface waters of the State, pursuant to the application received October 10, 1983 and in conformity with the project plans, specifications, and other supporting data, subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This Permit shall be effective from the date of issuance until September 30, 1987 and shall be subject to the following specified conditions and limitations: 1. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 2. The sewage and wastewater collected by this system shall be adequately treated prior to spray irrigation and shall not result in a discharge to the surface waters of the State. • 3. This permit is not transferable. 4. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes, and may be rescinded unless the facilities are maintained and operated in a manner which will protect the assigned quality standards of the surface waters and ground waters. r • Permit No. 1477R Page 2 5. In event the facilities fail to perform satisfactorily including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal facilities. 6. The sludge generated from these treatment facilities must be disposed of in accordance with General Statutes 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. 7. A suitable year round vegetative cover shall be maintained. 8. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. 9. Dikes shall be constructed where necessary to prevent surface water runoff from the spray field. 10. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. • 11. Any monitoring deemed necessary by the Division of Environmental Management to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. a. Soil in the spray irrigation area shall be tested semi-annually for sodium content and the results submitted to the Division's Winston-Salem Regional office. b. A recorded log of gypsum applications to the sprayfield, including amounts applied and dates, shall be maintained by the Permittee as a permanent record and made available to the Division upon re- quest. 12. The sludge generated from these treatment facilities must.be disposed of in accordance with General Statutes 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. 13. Monitoring wells shall be constructed and sampling schedules arranged as required by the Division of Environmental Management and constructed in accordance with 10 NCAC 2C 0.100. a. Three (3) monitoring wells shall be installed at locations determined in consultation with the Division's Winston-Salem Regional Office hydrologist and shall be screened from 15 to 20 feet below land surface. . The wells shall be sampled at quarterly intervals for the following paramaters: NO2, NH3, Na, Cl, SO4 total organic carbon, total dissolved solids, conductance, pH and water level. (Note: Water level measurements should be made immediately prior to pumping or bailing for water quality sample. 14. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or ground waters resulting from the operation of this facility. 15. The Permittee shall employ a certified wastewater treatment plant operator in responsbile charge of the wastewater treatment facilities. Such operator must hold a certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. 16. The Permittee, at least six (6) months prior to the expiration of this Permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if indicated, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate.
