Division of Water Resources' Central and Regional staff has reviewed the application package
received February 28, 2020 and additional information received May 19, 2020. However, additional
information is required before the review may be completed. Please address the items on the attached pages
no later than the close of business on September 4, 2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through C, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

A. Site May:
The site map submitted in response to item A.2. of the May 19, 2020 additional information letter
by Tessa Monday does not appear to be up-to-date with site conditions. From aerial photography,
there are indications of sprinkler wetted areas where there are no spray field noted in this map (most
notably located directly north of the infiltration basins and east of the three -celled facultative
lagoon). There are also sections of "Existing Sprayfields", which are located on parcel 773-116.1
owned by Topsail Landing, LLC, which would not be allowed without an easement.
Please provide an up-to-date map. This map shall include:
a. Location of monitoring wells;
b. Location of all surface waters (including ponds, ditches, streams and other surface drainage
features) within 500 feet of all treatment, storage and disposal sites;
c. The wetted perimeter of all irrigation fields with each individual field delineated and named
according to the approved permit; and
d. A delineation of compliance and review boundaries for both the infiltration basins,
irrigation fields, and any unlined earthen storage/treatment structures.
2. If there are existing sprayfields located on parcel 773-116.1, which are not owned by the Permittee
as indicated by the submitted site map, an easement shall be submitted giving the Permittee
permission to be able to use the parcel for waste disposal activities. This easement shall convey to
the Permittee the authorization to operate, inspect, maintain, repair, and establish monitoring
activities on the parcel necessary for the function and evaluation of the fields.
B. GeneralOuestions:
1. Please clarify whether the 42.2 acre three -cell facultative lagoon and 9.5 acre dewatering storage
lagoon listed in the facility description are lined or unlined. If they are lined, please state the liner
type.
C. Groundwater Monitoring:
1. Table IV.1. of the Non -Discharge System Renewal form (NDSR 09-19) requires the gradient and
location of all wells relative to the compliance and review boundary. Please submit this
information.
2. As per the additional information response by Randy Hoffinan on May 19, 2020, it was indicated
that MW-8 was installed for groundwater monitoring purposes in the permit. This well needs to be
added to the current permit. Please submit the Well Construction Record Form (GW-1) for this new
well.

3. As stated in the previous additional information request, monitoring wells MW-3 and MW-7 have
recurrent ammonia exceedances. Based on these routine exceedances of ammonia at the review
boundary, we believe that it is necessary to establish groundwater monitoring downgradient of
wells MW-3 and MW-7 at the compliance boundary of the system in accordance with 15A NCAC
02L .0110(a).
The Division requests that the Permittee propose the locations for at least two additional wells
located on the compliance boundaries downgradient from well MW-3 and MW-7 (one additional
well for each existing wells). These proposed well locations shall be included on a site map with a
clearly delineated compliance and review boundary, and be approved by the NC DWR Wilmington
Regional Office prior to construction.
