Hi Karen,
I apologize for not getting this information to you sooner,but the following is what we talked about on the phone last
week:
a) Permit Coverage—Apex Nurseries is currently covered under the NCG210000 General Stormwater Permit for
Timber Products via three(3)Certificates of Coverage (COC):NCG210436,NCG210437,and NCG210439.
i. NCG210436 is for parcel# 17828. In our records,we labeled this Site 1.
ii. NCG210437 is for parcel# 17795. We labeled this Site 2.
iii. NCG210439 is for parcel# 79696. We labeled this Site 3.
If you click on the blue COC numbers,you'll be taken to our public records repository and can see the specific
documentation associated with each one. Probably most helpful to you will be to look at the original Notice of
Intent(NOI)applications.
The NCG210000 General Stormwater Permit for Timber Products and associated monitoring forms are available

b) Past Due Fees—In order for Apex's permit coverage to be active, all fees must be paid. Each COC has a$100
annual fee associated with it. Our records indicate that Apex failed to pay the annual fees from 2017, 2018,and
2019. Attached to this email is a PDF containing the nine invoices for past due fees for the three COCs {3 COCs
x 3 years of fees= 9 invoices{. You only need to write one check for the total amount and our billing specialist
will apply the money accordingly.
c) Combining COCs—Last year,the Stormwater Program was asked if any of the COCs could be combined or
eliminated. A COC can only cover contiguous areas; so after reviewing the maps and aerial images,we
determined that NCG210436 and NCG210439 could be combined into one COC because those parcels are
contiguous. It is not a requirement that these COCs be combined.
d) Stormwater Monitoring—As I explained on the phone,the Stormwater Program is transitioning to a more
streamlined submission process for DMRs. Going forward,this is what you will do with your monitoring forms:
i. Scan and upload signed DMRs directly into the appropriate COC file using the following link:
https://edocs.deg.nc.gov/Forms/SW-DMR
ii. Once you have uploaded the digital copy,mail the paper copy to the Raleigh Regional Office at 3800
Barrett Drive, Raleigh,NC 27609.
Now that you know which parcel goes with which COC,you should be able to determine which outfalls go with
which COC for monitoring purposes. If you're still unclear,please contact me or Thad.
