Howes, Secretary April 27, 1993 The Honorable Joe Comer, Mayor Town of Ellerbe 116 West Page Street Ellerbe, NC 28338 Subject: Application No. WQ0007820 Additional Information Request Town of Ellerbe Sewer Extension Richmond County Dear Mayor Comer: The Permits and Engineering Unit has completed a preliminary engineering review of the subject application. The following items must be addressed before we can complete our review: 1) Please fill out item No. 11 on page 4 of your permit application. Please be advised that on February 1, 1993, the Division adopted new regulations that require that the infiltration/exfiltration rate must not exceed 100 GPD per inch of pipe diameter per mile of pipe. All sewer extension permit applications received on or after February 1, 1993, must conform to these new regulations. Please resubmit three revised, signed and sealed copies of page 4 of the application form reflecting these changes. Please be advised that the Division is in the process of revising the application forms to reflect the new regulations. Please also revise section 02665-3 of your specifications to reflect this change. If you would like to send three revised copies of this page I would be happy to replace this page in your specifications, granted I have your permission. If you choose to have me replace this page please punch the necessary holes along the margin. 2) Please send the Division a copy of page 9 of your permit application signed by Mayor Comer under Applicant's Certification. Please resubmit three revised, signed and sealed copies of this page. 3) Please recheck the calculations for the total linear feet of 8 inch sewer line and 4 inch force main. If corrections need to be made, please resubmit three revised, signed and sealed copies of page 3 of the application form. Box 29535, Raleigh, North Carolina 27626-0535 Telephone 919-733-7015 An Equal Opportunity Affirmative Action Employer 4) Please show where the water line crosses the sewer line on the sewer profile so the Division can confirm that the 18 inch minimum vertical separation is maintained in the following areas; between MH #2 and MH #3, between MH #4 and MH #5 and between MH #3 and MH #9. 5) Effective February 1, 1993, 15A NCAC 2H .0219 (h) (3) requires that "at least one of the following shall be required for pump stations: (A) dual source or standby power supply on site or, (B) telemetry systems with sufficient numbers of standby generators and personnel for distribution or, (C) approval by the Director that the pump station: i) serves a private water distribution system which has automatic shutoff at power failure and no elevated water storage tanks, and ii) has sufficient storage capacity that no potential for overflow exists, and iii) is connected to facilities that can tolerate septic wastewater due to prolonged detention or, (D) where the waters that would be impacted by a power failure are classified as C, the applicant may be allowed to show a history of power reliability that would demonstrate that an alternative power source or other reliability measures would not be needed. Page 5 of your permit application indicates that an alternate power source is provided. If this alternate power source is a portable generator supplied by the Town of Ellerbe please provide additional information stating how many portable generators the town has and how many pump stations they are expected to serve in the event of a power failure. If the alternate power source is not a portable generator please provide a description of the alternate source. Refer to the subject permit application number when providing the requested information. Please submit three copies of all information to my attention at the address below. Also, please note that failure to provide this additional information on or before May 27, 1993 will subject your application to being returned as incomplete, in accordance with 15A NCAC 2H .0208.
