In accordance with your completed application to be covered under the State Stormwater General
Permit Number SWG040000 for a Linear Utility Line project, received on May 20, 2020, we are
forwarding herewith the subject Certificate of Coverage Number SWG040119 for the construction
of a linear utility line project with associated incidental built -upon area. The General Permit is
issued pursuant to the requirements of North Carolina General Statute 143-215.1 and Title 15A
NCAC 2H .1000, the stormwater management rules.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the
conditions and limitations as specified therein, and does not affect the legal requirement to obtain
other permits which may be required by the Division of Water Quality, the Division of Land
Resources, Coastal Area Management Act, or any other federal, state, or local agency, law, rule, or
ordinance.
Please take notice that this Certificate of Coverage is not transferable except by action of the
Division of Environmental Quality (DEQ). DEQ may require modification or revocation and
reissuance of the Certificate of Coverage.
This project will be kept on file at the Winston-Salem Regional Office. If you have any questions, or
