Please verify if a Representative Outfall Status Request is needed for the three outfalls at the Conestoga Wood
Specialties facility in Johnston County. If so, I've provided a hyperlink to the form below.
Request Representative Outfall Status
Complete the Representative Outfall Status (ROS) Request Form. Mail one hard copy of the form and
supporting documents directly to the appropriate Regional Office (see form). In addition, e-mail the form and
supporting documents to Suzanne McCoy. There is no fee for a representative outfall status request.

I am in the process of updating the Outfall information for the stormwater certificate of coverage NCG180021.
You had indicated that outfall 003 required a Representative Outfall Status, please verify if you also require a
Representative Outfall Status for 001 and 002.
