In accordance with your permit major modification request received April 17, 2020, and subsequent
additional information received July 17, 2020, we are forwarding herewith a modified Attachment B
certified August 3, 2020, to Synagro Central, LLC for the subject residuals management program.
Modifications to the subject Attachment are as follows: Addition of a residual source Killian Creek
WWTP (Permit No. NC0088722) with total maximum dry tons of 250 per year. Addition of 432.90 acres
of residual land application area in the following counties:
• Iredell County: Fields IR-36-01, IR-36-02, IR-36-03, IR-36-04 and IR-36-05 totaling 159.20
acres
• Rowan County: Fields RA-05-01, RA-05-02, RA-05-03, RA-53-01 and RA-53-02 totaling
121.20 acres.
• Alexander County: Fields AL-13-04, AL-13-05, AL-13-06, AL-15-01, AL-15-02, AL-15-03
and AL-16-01 totaling 118.50 acres
• Caldwell County: Fields CD-03-02, CD-03-03, CD-03-06, and CD-03-07 totaling 34 acres.
The modified Attachments A and B shall be effective from the date of certification until October
31, 2022 (i.e., expiration date of the current permit), shall void Attachment A certified May 16, 2019 and
shall void Attachment B of Permit No. WQ0016247 issued November 17, 2017 and shall be subject to the
conditions and limitations as specified therein, as well as those specified in the above -referenced permit.
Please replace Attachment A certified May 16, 2019 and replace Attachment B of Permit No. WQ0016247
that was issued November 17, 2017 with the modified Attachments A and B. Please note that the
Permittee shall submit a renewal application no later than May 4, 2022.
Please note that only the maps for new land application sites are attached to this letter. It is the
Permittee's responsibility to ensure that a complete permit is created by inserting these documents into the
previously issued permit.

If any parts, requirements or limitations contained in this certification are unacceptable, the
Permittee has the right to request an adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.

1. Maximum Dry Tons per Year is the amount of residuals approved for land application from each permitted facility.
2. Analyses to demonstrate that residuals are non -hazardous (i.e., TCLP, ignitability, reactivity, and coirosivity) as stipulated under permit Condition IV.2.
3. Testing of metals and nutrients as stipulated under permit Condition IV. 3.
4. Analyses of pathogen and vector attraction reductions as stipulated under permit Condition IV. 4.
5. Monitoring frequencies are based on the actual dry tons applied per year using the table below, unless specified above.

6. Class A water treatment plant residuals are exempt from meeting the pathogen and vector attraction reduction requirements in Condition IVA, provided they are not mixed with residuals that have
been generated during the treatment of domestic wastewater, the treatment of animal processing wastewater, or the biological treatment of industrial wastewater. The Class A water treatment plant
residuals must be treated and processed in a manner as to not meet the definition of "Biological Residuals" as defined in 15A NCAC 02T .1102(6).
If no land application events occur during a required sampling period (e.g., no land application occur during an entire year when annual monitoring is required), then no sampling data is
required during the period of inactivity. The annual report shall include an explanation for missing sampling data. Those required to submit the annual report to United States Environmental
Protection Agency (US EPA) may be required to make up the missed sampling. Contact the US EPA for additional information.
