On July 30, 2020, I accompanied Caitlin Caudle from our office to the subject facility. Caitlin conducted a routine compliance inspection and we also reviewed the WWTP in comparison to the updated system plans provided in the add info response. Some items were noted which were not specifically included in the original staff report. Below is a summary of these items along with recommendations for the new permit's system description and schedule conditions. 1. There is an influent flow meter present on at the head of the WWTP on the subject system that is not listed in the system description contained in the current permit. Please add the "AMETEK Ultra Sonic influent flow meter" to the new permits system description. 2. There is a sludge storage basin present on the subject system that is not listed in the system description contained in the current permit. Please add the "approximately 3,500 gallon sludge storage basin" to the new permits system description. 3. The initial splitter box at the head of the WWTP has old lines present (3 old lines observed) that need to be closed off. These lines were previously used as part of the industrial treatment system and are no longer needed. However, during high flow events raw domestic wastes is overflowing in the splitter box and entering these old lines, one of which goes directly to the effluent storage pond with no treatment. Under the old permit and original design, these lines were necessary, but with the updated plans provide in the add info response these lines are to be abandoned to prevent bypasses from occurring. Please add a schedule condition that requires all lines not in use with the domestic treatment system to be closed off to influent flow within 30 days of permit issuance. 4. The sludge storage basin has old lines present (2 old lines observed) that need to be closed off. These lines were previously used as part of the industrial treatment system and also contained a bypass line directly to the effluent storage pond. According to the updated plans provided in the add info response the only pipes needed now are the input line from the domestic WWTP and a decant line. Please add a schedule condition that requires all the old lines not in use with the domestic treatment system to be closed off within 30 days of permit issuance. 5. The transfer pipe from the "new pump building" which is intended to transfer treated effluent from the 588,000-gal storage pond to the recently certified 2,005,265-gal effluent storage lagoon is not connected. Thus, not allowing any storage or use of the 2,005,265-gal lagoon. This has resulted in at least one recent event with inadequate freeboard issues which required effluent pump & haul operations. Please add a schedule condition that this pipe be installed within 60 days of permit issuance. 6. Please add a schedule condition that requires an Engineer to certify construction items #3, #4 and #5 listed above. 7. Onsite representatives indicated that the Engineer that supplied the recent add info response is putting together a revised O&M Plan (including residuals management & spill response). Please add a schedule condition that requires the revised O&M Plan be submitted to the Division within 90 days of permit issuance.
