-'` A letter of request for an Authorization to Construct was received November 9,
1987, by the Division and final plans and specifications for the subject project have
been reviewed and found to be satisfactory. Authorization is hereby granted for the
construction of a .008. MGD wastewater treatment facility consisting of a bar screen, a
8, 050 gallon aeration tank .with_ 42 CFM each dual. blowers, _a -1,535_gallon _s-econdarX_
clarifier, a 1,400 gallon aerated sludge holding tank., a 167 gallon chlorine contact
-tank equipped with a tablet type chlorinator,_- a 1550 linear:. feet- of--i 1/2-.-inch _ -
effluent. force main, a 15 GPM wet well type pump station equipped with simplex pump
and high water alarm, all related valves pipes and appurtenances'to serve Twin Oaks -
Apartments located in Henderson County.
This Authorization to Construct is issued —in: accordance with Part III.paragraph C -
of the NPDES Permit No. NC0066362 issued November 1, 1986, and shall -be subject to ---
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NC0066362.
The permittee must employ a certified wastewater operator in accordance with Part
III paragraph D of the referenced permit. -

accordance with G.S. 143-215.1 and in a manner approvable by the North--Car-elina-
Division of Environmental Management. -

In event the facilities fail to perform satisfactorily in meeting its NPDES _
permit effluent limits, the Permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional wastewater
treatment and disposal facilities.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received -from a professional engineer certifying that the permitted facility has been installed in accordance -with the 6pproved plans
and specifications.
