Dear Mr. Gilmore:
You have our approval, in accordance with the attached conditions for the purpose of constructing a bridge, as you
described in your application dated November 16, 1998. After reviewing your application, we have decided that this fill
is covered by General Water Quality Certification Number 3114. This Certification allows you to use Nationwide Permit
Number 33 when the Corps of Engineers issues it. In addition, you should get any other federal, state or local permits
before you go ahead with your project including (but not limited to) Sediment and Erosion Control, Coastal Stormwater,
Non-Discharge and Water Supply Watershed regulations. Also this approval will expire when the accompanying 404 or
CAMA permit expires unless otherwise specified in the General Certification.
This approval is only valid for the purpose and design that you described in your application. If you change your
project, you must notify us and you may be required to send us a new application for a new certification. If the property
is sold, the new owner must be given a copy of the Certification and approval letter and is thereby responsible for
complying with all conditions. If total wetland fills for this project (now or in the future) exceed one acre, compensatory
mitigation may be required as described in 15A NCAC 2H.0506 (h). For this approval to be valid, you must follow the
conditions listed in the attached certification.
If you do not accept any of the conditions of this certification, you may ask for an adjudicatory hearing. You must
act within 60 days of the date that you receive this letter. To ask for a hearing, send a written petition which conforms to
Chapter 150B of the North Carolina General Statutes to the Office of Administrative Hearings, P.O. Box 27447, Raleigh,
N.C. 276 1 1-7447. This certification and its conditions are final and binding unless you ask for a hearing.
This letter completes the review of the Division of Water Quality under Section 401 of the Clean Water Act. If you

SUBJECT: APPLICATION FOR A NATIONWIDE 33 PERMIT FOR BRIDGE NO.
104 OVER GREEN RIVER ON SR 1151. POLK COUNTY. TIP NO. 13-
2604.
On June 15, 1998 the US Army Corps of Engineers (USACE) issued a Section
404 Nationwide Permits 23 and 33, Action ID # 199830829 and 199830830. The
information regarding the project has not changed since the distribution of these permits.
The bridge will be replaced on new location just north of the existing structure. Traffic
will be maintained on Bridge No. 105 during construction.
It will be necessary to construct a temporary crossing of the river for project
construction since the existing bridge does not have the capacity to support the
construction equipment. NCDOT has designed a temporary bridge to facilitate this
crossing. Construction of the temporary bridge will result in approximately 0.06 acre of
temporary fill in surface waters. The fill material will consist of washed Class II Rip
Rap and washed Class "B" Rip Rap. Upon completion of the project, the fill will be
removed to the extent practicable without disturbing the river bottom. The bridge will be
in place for approximately six months. Drawings of the bridge are attached.
It is anticipated that these activities will be authorized under Section 404
Nationwide Permit 33 (Temporary Construction Access and Dewatering) and NCDOT is
requesting the issuance of the permit. By copy of this letter, it is also requested that a
General Water Quality Certification 43114 be issued by the North Carolina Division of
Water Quality. If you have any questions or need any additional information, please
