Division of Water Resources INSTRUCTIONS FOR FORM: FTA 04-16 & SUPPORTING DOCUMENTATION
This application is for sewer extensions involving gravity sewers, pump stations and force mains, or arty combination that has been
certified by a professional engineer and the applicant that the project meets the requirements of 15A ,1CA�2r and the Division's
Minimum Design Criteria and that plans, specifications and supporting documents have been prepared In accordance with, 15A
NCAC 02T, 15A NCAC 02T .030% Division policies and -ood en ineerirt practices.

While no upfront engineering design documents are required for submittal, in accordance with 15A NCAC 02T .0305(b), design
documents must be prepared prior to submittal of a fast track permit application to the Division. This would include plans, design
calculations, and project specifications referenced in 15A NCAC 02T .0305 and the applicable minimum design criteria. These
documents shall be available upon request by the Division.
Projects that are deemed permitted (do not require a permit from the Division) are explained in 15 CAC 02T.0303.
Projects not eligible for review via the fast track process (must be submitted for full technical review):
➢ Projects that require an environmental assessment in accordance with 15A NCAC 1 C .0100;
➢ Projects that do not meet any part of the minimum design criteria (MDC) document;
➢ Projects that involve a variance from the requirements of 15A NCAC 2T;
D Pressure sewer systems utilizing septic tank -effluent pumps (STEPs) or simplex grinder pumps;
D STEP or simplex grinder pumps connecting to pressurized systems (e.g. force mains);
➢ Vacuum sewer systems.
General - When submitting an application, please use the following instructions as a checklist in order to ensure all required items
are submitted. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and
reduce the amount of requested additional information. Failure to submit all required items will necessitate additional
processing and review time, and may result in return of the application. Unless otherwise noted, the Applicant shall submit
one original and one copy of the application and supporting documentation.
A. One Original and One Copy of Application and Supporting Documents

B. Cover Letter (Required for All Application Packages):
® List all items included in the application package, as well as a brief description of the requested permitting action.
Be specific as to the system type, number of homes served, flow allocation required, etc.
➢ If necessary for clarity, include attachments to the application form.
C. Application Fee (All New and Major Modification Application Packages):
® Submit a check or money order in the amount of $480.00 dated within 90 days of application submittal.
Payable to North Carolina Department of Environmental Quality (NCDEQ)
D. Fast Track (Form: FTA 04-16) Application (Required for All Application Packages):
® Submit the completed and appropriately executed application.
➢ If necessary for clarity or due to space restrictions, attachments to the application may be made.
® If the Applicant Type in Item I.2 is a corporation or company, provide documentation it is registered for business with
the North Carolina Secretan of State.
El If the Applicant Type in Item 1.2 is a partnership or d/b/a, enclose a copy of the certificate filed with the Register of
Deeds in the county of business.
® The Project Name in Item 11.1 shall be consistent with the project name on the flow acceptance letters, agreements, etc.
® The Professional Engineer's Certification on Page 5 of the application shall be signed, sealed and dated by a North
Carolina licensed Professional Engineer.
® The Applicant's Certification on Page 5 of the application shall be signed in accordance with 15A NCAC 02T _0106 1 b.
Per 15A NCAC 02T .0106(c), an alternate person may be designated as the sighing official if a delegation letter is
provided from a person who meets the criteria in 15A NCAC 02T .0106(b).

E. Flow Tracking/Acceptance Form (Form: FTSE 04-16) (If Applicable):
® Submit the completed and executed FTSE form fium the owners of the downstream sewers and treatment facility.
➢ Multiple forms maybe required where the downstream sewer owner and wastewater treatment facility are different.
➢ The flow acceptance indicated in form FTSE must not expire prior to permit issuance and must be dated less than one
year prior to the application date.
➢ Submittal of this application and form FTSE indicates that owner has adequate capacity and will not violate G.S. 1 =3-
2 5.b74a.1.
D Intergovernmental agreements or other contracts will not be accepted in lieu of a project -specific FTSE.
F. Site Maps (All Application Packages):
® Submit an 8.5-inch x I 1-inch color copy of a USGS Topograpbic Map of sufficient scale to identify the entire project
area and closest surface waters.
D Location of the project (gravity sewer, pump stations & force main)
➢ Downstream connection points and permit number (if known) for the receiving sewer
® Include a street level map (aerial) showing general project area so that Division staff can easily locate it in the field.
G. Existing permit (All Modification Packages):
® Submit the most recently issued existing permit.
® Provide a list of any items within the permit the Applicant would like the Division to address during the permit
modification (i.e., permit description, flow allocation, treatment facility, etc.).
H. Power Reliability Plan (Required ifportable reliability option utilized for Pump Station):
❑ Per 15A NCAC 2T .0305f h)t1 I, submit documentation of power reliability for pumping stations.
➢ This alternative is only available for average daily flows less than 15,000 gallons per day
D It shall be demonstrated to the Division that the portable source is owned or contracted by the applicant and is compatible
with the station. The Division will accept a letter signed by the applicant (see I SA NCAC 02T .0106(b)) or proposed
contractor, stating that "the portable power generation unit orportable, independently -powered pumping units, associated
appurtenances and personnel are available for distribution and operation of this pump station."
➢ If the portable power source or pump is dedicated to multiple pump stations, an evaluation of all the pump
stations' storage capacities and the rotation schedule of the portable power source or pump, including travel
timeframes, shall be provided In the case of a multiple station power outage. (Required at time of certification)
I. Certificate of Public Convenience and Necessity (AH Application Packages for Privately -Owned Public Utilities):

In accordance with your application received May 6, 2019, and additional Information received on June
19, 2019, we are forwarding herewith Permit No. WQ0040955 dated June 20, 2019, to the Town of
Pittsboro (Permittee) for the construction and o eration upon certification of the subject wastewater
collection system extension. This permit shall be effective from the date of issuance until rescinded and
shall be subject to the conditions and limitations as specified therein. This cover letter shall be considered
a part of this permit and is therefore incorporated therein by reference.
Please pay particular attention to the following conditions contained within this permit:
Condition 11.1: This permit shall not be automatically transferable; a request must be made and
approved.
Condition 11.4: Requires that the wastewater collection facilities be properly operated and maintained
in accordance with 15A NCAC 02T .0403 or any individual system -wide collection system
permit issued to the Permittee.

Town of Pittsboro is hereby authorized to:
Construct, and then operate upon certification the aforementioned wastewater collection extension. The
sewage and wastewater collected by this system shall be treated in the Town of Pittsboro's Wastewater
Treatment Facility in accordance with Permit Number NC0020354.
Permitting of this project does not constitute an acceptance of any part of the project that does not meet
15A NCAC 02T; the Division's Gravity Sewer Minimum Design Criteria adopted February 12, 1996 as
applicable; and the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump Stations and
Force Mains adopted June 1, 2000 as applicable, unless specifically mentioned herein. Division approval
is based on acceptance of the certification provided by a North Carolina -licensed Professional Engineer in
the application. It shall be the Permittee's responsibility to ensure that the as -constructed project meets
the appropriate design criteria and rules.
Construction and operation is contingent upon compliance with the Standard Conditions and any Special
Conditions identified below.
I. SPECIAL CONDITIONS
NONE
11. STANDARD CONDITIONS
1. This permit shall not be transferable. In the event there is a desire for the wastewater collection
facilities to change ownership, or there is a name change of the Permittee, a formal permit request
shall be submitted to the Division accompanied by documentation from the parties Involved, and
other supporting materials as may be appropriate. The approval of this request shall be considered
on its merits and may or may not be approved. 115A NCAC 02T.0116; G.S 143-215.1(d3)i
2. This permit shall become voidable unless the wastewater collection facilities are constructed in
accordance with the conditions of this permit; 15A NCAC 02T; the Division's Gravity Sewer Minimum
Design Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for
the Fast -Track Permitting of Pump Stations and Force Mains adopted June 1, 2000 as applicable; and
other supporting materials unless specifically mentioned herein. [15A NCAC 02T.01101
3. This permit shall be effective only with respect to the nature and volume of wastes described in the
application and other supporting data. (I5A NCAC 02T .0110j
4. The wastewater collection facilities shall be properly maintained and operated at all times. The
Permittee shall maintain compliance with an individual system -wide collection system permit for the
operation and maintenance of these facilities as required by 15A NCAC 02T .0403. If an individual
permit is not required, the following performance criteria shall be met:
a. The sewer system shall be effectively maintained and operated at all times to prevent discharge
to land or surface waters, and to prevent any contravention of groundwater standards or surface
water standards.

b. A map of the sewer system shall be developed and shall be actively maintained.
c. An operation and maintenance plan including pump station inspection frequency, preventative
maintenance schedule, spare parts inventory and overflow response has been developed and
implemented.
d. Pump stations that are not connected to a telemetry system shall be inspected every day (i.e. 365
days per year). Pump stations that are connected to a telemetry system shall be inspected at
least once per week.
e. High -priority sewer lines shall be inspected at least once per every six -months and inspections are
documented.
f. A general observation of the entire sewer system shall be conducted at least once per year.
g. Overflows and bypasses shall be reported to the appropriate Division regional office in accordance
with 15A NCAC 28 .0506(a), and public notice shall be provided as required by North Carolina
General Statute §143-215.1C.
h. A Grease Control Program is in place as follows:
1. For public owned collection systems, the Grease Control Program shall include at least
biannual distribution of educational materials for both commercial and residential users and
the legal means to require grease interceptors at existing establishments. The plan shall also
include legal means for inspections of the grease Interceptors, enforcement for violators and
the legal means to control grease entering the system from other public and private satellite
sewer systems.
2. For privately owned collection systems, the Grease Control Program shall include at least bi-
annual distribution of grease education materials to users of the collection system by the
permittee or its representative.
3. Grease education materials shall be distributed more often than required in Parts (1) and (2)
of this Subparagraph if necessary to prevent grease -related sanitary sewer overflows.
1. Right-of-ways and easements shall be maintained in the full easement width for personnel and
equipment accessibility.
j. Documentation shall be kept for Subparagraphs (a) through (1) of this Rule for a minimum of three
years with exception of the map, which shall be maintained for the life of the system.
5. The Permittee shall report by telephone to a water resources staff member at the Raleigh Regional
Office, telephone number (919) 791-4200, as soon as possible, but in no case more than 24 hours,
following the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable
of adequate wastewater transport, such as mechanical or electrical failures of pumps, line
blockage or breakage, etc.; or
b. Any SSO and/or spill over 1,000 gallons; or
c. Any SSO and/or spill, regardless of volume, that reaches surface water
Voice mail messages or faxed information is permissible, but this shall not be considered as the initial
verbal report. Overflows and spills occurring outside normal business hours may also be reported to
the Division of Emergency Management at telephone number (800) 858-0368 or (919) 733-3300.
Persons reporting any of the above occurrences shall file a spill report by completing and submitting
Part I of Form CS-SSO (or the most current Division approved form) within five days following first

knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to
ensure that the problem does not recur. Part II of Form CS-SSO (or the most current Division approved
form) can also be completed to show that the SSO was beyond control. [G.S.143-215.iC(al))
6. Construction of the gravity sewers, pump stations, and force mains shall be scheduled so as not to
Interrupt service by the existing utilities nor result in an overflow or bypass discharge of wastewater
to the surface waters of the State. [15A NCAC 02T.0108(b))
7. Upon completion of construction and prior to operation of these permitted facilities, the completed
Engineering Certification form with checklist attached to this permit shall be submitted with the
required supporting documents to the address provided on the form. A complete certification is one
where the form Is fully executed and the supporting documents are provided as applicable. Any
wastewater flow made tributary to the wastewater collection system extension prior to completion
of this Engineer's Certification shall be considered a violation of the permit and shall subject the
Permittee to appropriate enforcement actions.
If the permit is issued to a private entity with an Operational Agreement, then a copy of the Articles
of Incorporation, Declarations/Covenants/Restrictions, and Bylaws that have been appropriately
filed with the applicable County's Register of deeds office shall be submitted with the certification.
A complete certification is one where the form is fully executed and the supporting documents are
provided as applicable. Supporting documentation shall include the following:
a. One copy of the project construction record drawings (plan & profile views of sewer lines & force
mains) of the wastewater collection system extension. Final record drawings should be clear on
the plans or on digital media (CD or DVD disk) and are defined as the design drawings that are
marked up or annotated with after construction information and show required buffers,
separation distances, material changes, etc.
b. One copy of the supporting applicable design calculations including pipe and pump sizing, velocity,
pump cycle times, and level control settings, pump station buoyancy, wet well storage, surge
protection, detention time in the wet well, and force main, ability to flush low points in force
mains with a pump cycle, and downstream sewer capacity analysis. If a portable power source or
pump is dedicated to multiple stations, an evaluation of all the pump stations' storage capacities
and the rotation schedule of the portable power source or pump, include travel timeframes, shall
be provided.
c. Changes to the project that do not result in non-compliance with this permit, regulations, or the
Minimum Design Criteria should be clearly identified on the record drawings, on the certification
in the space provided, or in written summary form.
Prior to Certification (Final or Partial): Permit modifications are required for any changes resulting
in non-compliance with this permit (including but not limited to pipe length changes of 10% or
greater, Increased flow, pump station design capacity design increases of 5% or greater, and
increases in the number/type of connections), regulations, or the Minimum Design Criteria.
Requested modifications or variances to the Minimum Design Criteria will be reviewed on a case -by -
case basis and each on its own merit. Please note that variances to the Minimum Design Criteria
should be requested and approved during the permitting process prior to construction. After -

construction requests are discouraged by the Division and may not be approved, thus requiring
replacement or repair prior to certification & activation. [15A NCAC 02T .0116]
8. Gravity sewers installed greater than ten percent below the minimum required slope per the Divisions
Gravity Sewer Minimum Design Criteria shall not be acceptable and shall not be certified until
corrected. if there Is an unforeseen obstacle in the field where all viable solutions have been
examined, a slope variance can be requested from the Division with firm Supporting documentation.
This shall be done through a permit modification with fee. Such variance requests will be evaluated
on a case -by -case basis. Resolution of such request shall be evident priorto completing and submitting
the construction certification. [ 15A NCAC 02T.0105(n)]
9. A copy of the individual permit and construction record drawings shall be maintained on file by the
Permittee for the life of the wastewater collection facilities. [15A NCAC 02T .0116]
10. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 02T; the
Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable; the Division's
Minimum Design Criteria for the Fast -Track Permitting of Pump Station and Force Mains adopted June
1, 2000 as applicable; and other supporting materials may subject the Permittee to an enforcement
action by the Division, in accordance with North Carolina General Statutes §143-215.6A through §143-
215.6C, construction of additional or replacement wastewater collection facilities, and/or referral of
the North Carolina -licensed Professional Engineer to the licensing board. 115A NCAC 02T .0108(b-c)]
11. In the event that the wastewater collection facilities fail to perform satisfactorily, Including the
creation of nuisance conditions, the Permittee shall take immediate corrective action, including those
as may be required by this Division, such as the construction of additional or replacement facilities.

12. The issuance of this permit shall not exempt the Permittee from complying with any and all statutes,
rules, regulations, or ordinances that may be imposed by the Division any other Federal, State, or Local

Existing Pump Station Conditions
The existing CLLC pump station is outfitted with two (2) 25-HP pumps and a 6-foot
diameter wet well. The current pump capacity is approximately 200 gpm, and the flow is
regulated with mechanical float control. The existing pump station schematic can be
found in Figure A. The current pump station permit is shown in Attachment A.
Within the pump station's existing valve vault, two (2) existing 4-inch force mains are
routed from the existing wet well into one 6-inch force main. The 6-inch force main then
exits the pump station and Russet Run. This then ties into Powell Place and Lowes
before discharging into the gravity sewer at 15/501.
The pump station is currently outfitted with an odor control tank; however, the system is
not currently in use and there is no odor control at the pump station.
The pump station also is outfitted with an 80kW/100kVA generator. The generator's size
was the limiting factor for sizing pumps at the CLLC pump station.

The CLLC pump station is located in the sewer basin designated as G-01 in the Chatham
Park Wastewater Master PIan. The area of this basin is approximately 390 acres.
A loading value for a base flow of 99.09 gpm plus 140 gpm (totaling approximately 240
gpm) of wastewater was applied to the two (2) gravity sewers in G-01 to size the lines
and pump station in this area. The outline of basin G-01 and the locations of the loading
are shown in Figure B.

Proposed Pump Station Design
As noted above, future build -out, including a peak that is two (2) times that of the
average daily flow as required, will require a pump capable of 600 gpm. This will require
two (2) new 50 hp pumps capable of 600 gpm. The control system for the pump will be
upgraded to two (2) new VFDs (which will initially be configured and programmed to
operate and protect the existing 25 hp pump motors until the flow warrants the 50 hp
pumps). When the existing two (2) 25 hp pumps within the pump station are replaced by
two (2) new 50 hp motors and 600 gpm pumps, the control system will be switched from
floats to flow based controls for the new pumps. In order to allow for easier maintenance
and lifting of pumps, a floor mounted hoist with a 1000 lb load capacity will be installed.
The existing 80 kW generator will be kept. As required per state code, the generator will
power one pump at full loading.

This station is designed in accordance with the NCDENR criteria. The 600 gpm pump
will convey the peak hourly flow to its end point with the largest pump out of service.
These pumps are not designed to run in parallel and therefore the generator is not
required to provide power for both pumps. The pumps will be on a VFD that allows for
"considerations" to be made in the level control of the wet well, which is preexisting and
not being modified.
The valve vault inside the pump station will be upgraded and the force mains from the
wet well will be changed from two (2) 4-inch force mains to two (2) 8-inch force mains.
These force mains will exit the pump station structure and enter a proposed second valve
vault, where the two (2) 8-inch force mains will combine into one (1) 12-inch force main.
A 6-foot chain link fence around the pump station structure will be replaced and
expanded to include the new valve vaults as requested by Town of Pittsboro staff.
The 12-inch force main will then enter another proposed valve vault, which will tie the
new 12-inch force main to the existing force main. This 12-inch force main will then exit
the fenced -in pump station area and will be connected to the 12-inch force main line that
discharges into the existing gravity line flowing to the WRRF.
A 6-inch force main that runs approximately 415 feet will allow the existing Northwood
high school to reverse flow and be conveyed to CLLC PS. This will facilitate the startup
of the WRRF by providing additional flows.
These improvements are shown in Attachment B and summarized in Table A.

Construction Sequence
The CLLC pump station will remain in operation while improvements to the station are
underway. This requires that demolition and construction be carefully sequenced. This
sequencing will depend heavily upon the timing of other projects such as the
Decentralized Water Resource Reclamation Facility (DWRRF) and the gravity interceptor,
therefore, the sequencing suggestions in this report are general guidelines. The general
sequence of construction is as follows and is summarized in Figure E.
A new section of 6-inch force main from the existing line to the existing sanitary sewer
manhole will be constructed. While this part of the force main is being constructed, it is
expected that contractors would use the existing pump configuration until final tie-in.
This will include the valve vault, which allows flows in multiple directions.

This new force main configuration is meant to allow for construction of the new
discharge piping while maintaining the ability to continue pumping sewage towards
Powell Place. This built in flexibility allows, on a temporary basis, to direct the flows of
the Northwood High School to the manhole just upstream of the pump station. This
would allow those flows to be diverted towards the new DWRRF by closing the valve at

the intersection of 15/501 and Powell Place, to enable start-up of the DWRRF. A map of
this area is shown in Figure F.
The existing line that conveys flow from Northwood to the Town would remain without
flow for an extended period of time. Therefore, to minimize issues related to this
situation, we propose that potable water be used to flush the line immediately after the
diversion taking place.

The pump installation will be coordinated with the transfer of sewage from Northwood.
As noted above, the exact timing of the sequence depends on the timing of other projects.
The intent of the proposed design is to provide what is required to deliver service to the
area, while installation of the larger pumps is to be coordinated with the timing of the
opening of the DWRRF as well as with the consideration that the pumps be put into
service when the actual flows from Mosaic justify the larger pumps. This is to assure that
the pumps are put into service when installed so as not to waste the warranty period.
This sequencing will be closely coordinated with the Town of Pittsboro.
Future Pump Station Modifications
The initial master planning effort identified the area tributary to the CLLC pump station
as requiring 900 gpm (peak flow) to accommodate 518,400 gpd (ADF). Therefore, the
concurrent design for the CLLC pump station was intended as a temporary measure to
gain capacity for allocating sewer capacity and diverting flow to the new DWRRF and the
contemporaneous approved master plan reflected a larger pump station downstream
once development was underway.
However, design flow projections have become more granular and therefore more
accurate, it was determined that the proposed CLLC pump station design, capable of
handling a peak flow of 600 gpm to accommodate 390,600 gpd (ADF), was sufficient.
Therefore, no relocation or future increase in size is necessary based on current
development trends.

To complete the answer on 6a below: The information for the existing pump station was completely incorrect. I
received a copy of a report that was completed by McKim and Creed in January of 2019 that details the revisions to the
pump station that are currently being permitted. I haven't reached out to them yet to see when they think it will be
constructed yet but am planning on it. Regardless, the capacity of the pump station as it sits now is plenty for the
request that we are making. The design flow from the original permit application is 0.115 MGD (page 22 of the attached
document) and only there is only 0.006 MGD flow currently in the station. That leaves 0.091 MGD of available capacity
in its current state. According to the report, the 200 GPM pumps are slated to be replaced with 600 GPM pumps.
I hope this helps answer your questions. I am not sure where the original numbers that were shown on the FTSE came
from but they were completely incorrect (may have been left over from copying the form from another project).

We are following -up on the email Ted Cashion sent on May 161h regarding the referenced sewer extension
application. Ted had a few initial questions that were not answered, and after a more detailed review, we have more
questions. I will list them all here:
1. This application is a modification request to existing permit WQ0040955 issued on Jun3 20, 2019. The narrative
does not explain what is being modified. It appears that all the sewer line lengths are staying the same, but this
modification is to add flow to the original permit. Is that correct?
Yes this is a modification to add flow to the original permit. No line lengths are changing
2. Item V1.7 (flow calcs) shows 12 residential condo units @ 240 GPD. Are these all one or two bedroom units?
Most of them are 1 & 2 bedroom but there is one 3 bedroom unit that I will need to adjust the numbers for
will revise the application and submit a new one.
3, The narrative states that Phase 1 of this project include 47,950 GPD, and goes on to say the total estimated Daily
Design flow is 175,000 GPD. Will this additional flow be included in future applications?
Yes. The final number will likely be less than 275,000 GPD,
Item IX.6 has "No" checked for high priority lines. However, the submittal included a document for "25-year
Calcs for Aerial Sewer Crossing", so it appears that this project has an aerial crossing that would be a high -
priority line. Please address this.
I will revise the application to include the high -priority for the aerial crossing.
a. Also, Item IX.4 was checked "No" for 404/401 permit needed for this project. Since this project is almost
4,000 LF, and it appears it includes an aerial crossing of a stream, please confirm that a 404/401 permit
is not needed for any portion of this project.

Item Lf of the Flow -Tracking Sheet (FTSE) shows "0" flow for this request. This will need to be modified to show
the correct flow request.
The request has been sent to the Town and we will resubmit a revised FTSE form.
6. Item II of the Flow -Tracking (FTSE) appears to have some errors. It appears that the Firm Capacity (.012) and
Design Average Daily Flow (.030) are switched (Column A should be the firm capacity divided by the peaking
factor).
a. If these are switched to the correct columns, it appears that the CLLC PS may not have the capacity to
accept the new flow included in this modification application (Column E). Please address this.
I am working on addressing this and will have an answer when we resubmit.

We have received the subject application to modify permit WQ0040955. We have a few questions after a cursory
review of the application.
The FTSE form signed by Bryan Gruesbeck shows the total flow for this specific request as'0' (item Lf). According to the
application, 47,950 gpd will be tributary to the Pittsboro WWTP. Please have Bryan Gruesbeck complete and sign a
corrected FTSE form showing the correct flow for this request.
The USE form shows the CLLC PS with a firm capacity of 0.012 mgd, and a Design Average Daily Flow of 0.030 mgd. It
appears these values may be reversed. Please review and correct the values and calculations as needed.
Also, will any of the flow generated in this project be tributary to the Chatham Park WWTP now or in the future?
