Dear Sir:
SUBJECT: NATIONWIDE PERMIT 33 APPLICATION FOR BRIDGE NO. 80
OVER DEEP CREEK, PERSON COUNTY. TIP B-3015. ACTION II)
199602012.
On May 22, 1998 the U.S. Army Corps of Engineers issued a renewal of
Nationwide Permit No. 23 (Action ID. 199602012) for the subject project. It was
subsequently requested by the Contractor during a pre-construction meeting with
NCDOT, that they be allowed to place a temporary causeway across Deep Creek to
facilitate equipment crossing. Otherwise, the Contractor would have an approximately
4.5 mile long detour requiring overweight bridge crossing permits.
The causeway would be in place for approximately two weeks between late
September and early October. The causeway will consist of an 72 inch pipe back filled
with approximately 85 tons of either Class B rip-rap and/or clean broken-up concrete that
will fill approximately 300 ft' (0.01 acre) of the creek. A geotextile fabric will be placed
over the rip-rap and approximately six inches of ABC stone will cover the fabric. The 72
inch pipe is designed to carry the creek water even in the event of an unusually hard rain.
If the creek were to exceed this capacity, the water will run over the top of the causeway.
When equipment crossing is completed, the ABC stone, fabric, and pipe will be removed
in their entirety. The rip-rap/broken concrete will also be removed, except for an
approximate 12-inch layer. Removal of this layer would cause greater disturbance and
harm to the creek than leaving it in place. No wetlands will be impacted. Details of the
project are depicted in the attached drawings.

NCDOT has already designed and installed erosion control features for this
project. No additional devices are proposed.
Please review this permit application for approval under a Section 404
Nationwide Permit 33. By copy of this letter, it is also requested that the appropriate
General Water Quality Certification be issued by the North Carolina Division of Water
Quality. If you have any questions or need any additional information, please contact Mr.
Michael Wood at (919) 733-7844 extension 306.

CREEK CROSSING SUBM MAL
PERSON COUNTY BRIDGE PROJECT 99.2380501
September 1, 1998
1. DURATION: Only 3,750 cubic Yards of dirt must be moved across the creek. it should take no more
d= two weeks to perform the work.
2. TDAE OF YEAR: The work will be performed In late September or early October.
3. MATMUALS: We will install a 72 inch conduit in the existing creek to cary the water flow. This
pipe will be bank 01od with Class "13" rip rap of clean broken up concrete which will am allow some
water flow between voids. A running surface of ABC will bo spread over the top of the rip rap. The
ABC will be separated from the rip rat) with a layer of votextile fabric. This will serve to keep tb.
fines of the ABC from dropping into the voids of the rip rap. The 72" conduit should be large enough
m carry the creek water even in the event of a unusually hard rain. In the event however, that the
Week should well obovv aarmal !units, the waver will run over tho cop orate crossing taus prVivalting
any backup of water. The crossing will be approximately 12 feet wide and we anticipate the following
gttaiuities.
A. 72" Conduit 24 if
D. pip Rap SS tool
c, onotadie Fabric b5 square yard
D. ABC Stone 25 ton:
4. DISTURBANCE: Since tic dirt will be introduced to the creek, we feel very little erosion or
disturbance will be involved in the temporary crossing. Layinm the pipe and placing the initial lay n
rip rap in the creel? may cause suale silt to be stirred un Nola the creek bnttorrt but this should be
minor. Hauling over the creek crossius should not cause any orosion at all. Removal of the pipe and
rip rap should cause the same king of minor clouding as installation.
The Department of T ranspoatatacm has alroady designed adequate erosion features to oortumol the
arosion on the project. Since this crossing will cause no additional erosion, we no not think- additional
measums will be required.
c, pipe and most of the 7p
?.ESTORATTON: Once the dirt is hauled across the creek the ABC, fabri
-ap will be removes from the creek. In order to keep from disturbing the cleet, bottom, we may 1Ctve
a 12" layer of rip rap in the bottom. This layer will starve to both armor for the creek bottom and hop
reduce the disturbanvo that will be crralod during installation ou the new bridge.
