Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
This permit includes no major changes from the draft permit sent to you on March 2, 2005. If any
parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you
have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter.
This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General
Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina
27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal requirements
to obtain other permits which may be required by the Division of Water Quality or permits required by the Division
of Land Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended, the
County of Lincoln
is hereby authorized to discharge wastewater from a facility located at the
Middle Forney Creek WWTP
7533 South Little Egypt Road
Lincoln County
to receiving waters designated as Forney Creek in the Catawba River Basin
in accordance with effluent limitations, monitoring requirements, and other
conditions set forth in Parts I, II, III and IV hereof.
This permit shall become effective June 1, 2005.
This permit and authorization to discharge shall expire at midnight on February 28, 2010.

All previous NPDES Permits issued to this facility, whether for operation or
discharge are hereby revoked. As of this permit issuance, any previously issued
permit bearing this number is no longer effective. Therefore, the exclusive authority
to operate and discharge from this facility arises under the permit conditions,
requirements, terms, and provisions included herein.
The County of Lincoln is hereby authorized to:

1. Upstream = at least 50 feet upstream from the outfall. Downstream = at least 1 mile downstream from the
outfall. Instream samples shall be collected three times per week from June 1 - September 30, then
weekly for the remaining months of the year.
2. The monthly average effluent BOD and Total Suspended Solids Concentrations shall not exceed 15% of their
respective influent values (85% removal).
3. The weekly average values are three times the monthly average values (this value was established
based on a review of treatment capability at various municipal plants).
4. The daily average dissolved oxygen effluent concentration shall not be less than 5.0 mg/L.
5. The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

WWT facilities consist of an influent pump station with dual pumps, a Parshall
flume with ultrasonic flow measuring and recording, a mechanical bar screen with
a manual bypass screen, an aerated grit removal basin, and oxidation ditch with 3
channels, dual final clarifiers, dual chlorine contact chambers, and S02
dechlorination. Also in place is a rectangular effluent flow weir, cascade post
aeration, influent and effluent samplers, DAF sludge thickener, a return sludge
pumping station with dual pumps. The preexisting package plant was converted to
aerobic digestors with dual blowers, and the final clarifier was converted to a
gravity sludge thickener. The plant also has a caustic feed system with a
recirculation pump and dual feed pumps.

Middle Forney Creek W WTP is a .75 MGD wastewater facility consisting of an influent pump
station with dual pumps, a Parshall flume with ultrasonic flow measuring and recording, a mechanical bar
screen with a manual bypass screen, an aerated grit removal basin measuring 10 ft. X 10.5 ft. X 10 ft.
with a capacity of 7,854 gallons. An Oxidation ditch with a total dimension of 106.5 ft. X 124.5 ft. with 3
channels. Channel 1 measures 52 ft. X 15 ft. X 9 ft. with a capacity of 350,000 gallons. Channel 2
measures 36 ft. X 15 ft. X 9 ft. with a capacity of 250,000 gallons. Channel 3 measures 20 ft. X 15 ft. X
9 ft. with a capacity of 150.000 gallons. The 3 channels have a total of six rotor aerators. The plant has
dual final clarifers with a diameter of 35 ft.X 13.5 ft and a capacity of 97.104 gallons each, dual chlorine
contact chambers measuring 32.3 ft X 11.6 ft. X 6 ft. with a capacity of 16,918 gallons. and SO2
dechlorination. Also in place is a rectangular effluent flow weir, cascade post aeration. influent and
effluent samplers, a DAF sludge thickener which has never been in operation, a return sludge pumping
station with dual pumps. The pre-existing package plant was convened to aerobic digesters with dual
blowers and the final clarifer was convened to a gravity sludge thickener.The aerobic dibesters consists of
4 metal tanks, holding 10.500 gallons, 9627 gallons. 19.400 gallons and 121,445 gallons or a total of
160.972 gallons.. The thickened sludge pwnp station pumps to a sludge holding tank that measures
56 ft.diameter X 20 ft. with a capacity of 368.279 gallons. The sludge holding tank has a sludge
recirculating/truck loading pwnp station with dual pumps. The plant also has a caustic feed system with a
recirculation pump and dual feed pumps.

Sludge Management Plan
Prior to February 2000 our sludge was transported by pumper
trucks to the City of Lincolnton WWTP and applied to their drying
beds. Beginning in February 2000 Lincoln County contracted with
Southern Soil Builders of Roaring River, N. C. to land apply our
sludge. After a sludge analysis is performed and the sludge is
checked for a reduction in total volatile solids the sludge is pumped
from our sludge holding tank to Southern Soil Builder's tanker trucks,
which then transport the sludge to the appropriate field. There the
sludge is applied to the soil by Southern Soil Builder's equipment and
personnel.
