Sharp Lincoln County 115 West Main Street Lincolnton, NC 28092 Subject: Permit No. NCO074012 Authorization to Construct Middle Forney Creek Wastewater Treatment Plant Lincoln County Dear Mr. Sharp: A letter of request for Authorization to Construct was received December 2, 1988, by the Division and final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of a 35,000 GPD wastewater treatment plant consisting of an aerated 9,700 gallon flow equalization tank, a 35,000 gallon aeration tank, a 5,830 gallon clarifier, a 7,875 gallon aerated sludge holding tank, dual 175 CFi1 blowers, a tablet chlorinator, 1,456 gallon chlorine contact tank and a continuous, recording flow meter. This Authorization to Construct is issued in accordance with Part III, paragraph B of NPDES Permit No. NCO074012 issued November 11, 1988, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0074012. The Permittee must employ a certified wastewater operator in accordance with Part III paragraph C of the referenced permit. The sludge generated from these treatment facilities must be disposed of in accordance with General Statute 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. The Mooresville Regional Office, phone no. (704) 251-6208 shall be notified at least twenty-four (24) hours in advance of operation of the installed facility so that an in -place inspection can be made. Such notification to the regional supe=isor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. P.O. Box 27687, Raleigh, North Carolina 27611-7687 Telephone 919 733-7015 An Equal Oppommiry Affirmative Action Employer In the event the facilities fail to perform satisfactorily in meeting its NPDES permit effluent limits, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional wastewater treatment and disposal facilities. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the approved plans and specifications. Mail the certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, N.C. 27611-7687. A copy of the approved plans and specifications shall be maintained on file by the permittee for a minimum of five years from the date of issuance.
