This letter is written to provide a point-by-point response to Request for Additional
Information comments, which we received following the review of the 2nd Low Density
Stormwater Submission for the referenced project. Below are the comments received on the
8th of July 2020, each followed by our response in bold type.

NCDEQ Comments (Jim Farkas)

1. Original Comment 3 – “Please provide an original, signed in pen, hard copy of the
supplement form (Section V & Section VI, 3).” The provided draft copy of the O&M
Agreement Form is satisfactory. Please have the form signed & notarized and submit
the original hard copy with the next resubmission.

Response: Executed O& M agreement has been provided with this submittal.

2. Original Comment 7 – “If applicable, please show the surface waters on the plans
(Section VI, 8h) and the associated vegetated buffers (Section VI, 8p)… ensure
Section IV, 6-8 get updated.” Per the provided project area map, there appears to be
two surface waters located within the project area. One is Bakers Swamp that borders
the project area on the western property boundary (only a small amount of the
surface water appears to be located within the project area, but the vegetated buffer
should still be shown) and the other is an unnamed tributary to Barkers Swamp that
flows through the wooded area in the southern portion of the project. These surface
waters have been included on the plans, but the required vegetated buffer (15A
NCAC 02H .1017(10)) and the square footage of the surface waters (15A NCAC
02H .1003(1)(c)(i)) have not been shown/included. Please ensure that Section IV, 6-8
and the Low Density Supplement (Built-upon area calculation) are updated to reflect
the on-site surface waters. Since this project is located in a part of the State that is
subject to Phase II Post-Construction Stormwater Management and there are surface
waters on or around the project site, please ensure that Part IV, c of the Low Density
Supplement is initialed.

Response: Low Density Supplement and the Stormwater Application have
revised accordingly. The plans have been revised to include the 30’ Vegetated
Setback per our email correspondence.

3. Original Comment 8 – “Please provide the bearings and distances for the project area
boundary on the plans (Section VI, 8i).” Please provide the Project Area Exhibit in the
main set of plans. NOTE: For the resubmission, you can provide 1x hard copy (since I
already have one) of the Project Area Exhibit and I will incorporate them into the
provided plan sets.

Response: Project Area Exhibit has been included in the plan set. I went ahead
and included 2 new hard copies since the previous exhibit did not include the
Vegetated Setbacks on the streams.

4. Original Comment 11 – “The person signing Section IX of the Application, “Mary
Duane Gilliam Hale, Trustee”, does not match the Property Owner, “J. Duane Heirs
Gilliam”. Please correct this or provide official documentation showing that they may
sign on behalf of the Property Owner.” There appears to be a typo in the contact
information from item 2a in Section IX (“Filliam” is used instead of “Gilliam”).
NOTE: New original signatures are only required if information on the signature pages
has changed.

Response: Application has been corrected. No new signatures were required.

5. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of
other documents. Pdfs must be uploaded using the form at:
https://edocs.deq.nc.gov/Forms/SW_Project_Submittal

Response: All revised docs have been uploaded to the portal.
