Good morning!
Attached are: the original NOI for this project, ESC documents, and certification form — as well as the info about the
name change. Please fill out a new eNOI (using the copy attached as a guide to make it easier) and upload the
accompanying documents so we can issue you a new COC with the revised project name. You can include the letter
about the name change, too. We will waive payment and push the application through as soon as we can.

project Addison Fifteen 20 was not approved for the following reason:
Since coverage began only a month ago, and the change was made by the city, please submit a new eNOI
instead so the project name will be correct on a new COC. **We will waive the payment fee and issue a new
Certificate of Coverage.** Once the new one is issued, you can request a Notice of Termination for this COC. I
will follow up this email with a copy of the original NOI and supporting documents for upload.

As the authorized manager of the Addison Concord, LLC, we are writing to inform of
the above referenced projects name change. The project was originally filed under The
Addison Fifteen 20, but the project's name has changed based on addressing provided
by the City of Concord. The project shall be now known going forward as The Addison
Eighty 50. Please note there has been no change in ownership or management.
