Dear Mr. Frank Vick:
You have our approval, in accordance with the attached conditions and those listed below, for the
purpose of widening NC 904 at Grissettown, as you described in your application dated April 7, 1998. After
reviewing your application, we have decided that this impact is covered by General Water Quality
Certification Number 3103. This Certification allows you to use Nationwide Permit Number 14 when it is
issued by the Corps of Engineers. In addition, you should get any other federal, state or local permits before
you go ahead with your project including (but not limited to) Sediment and Erosion Control, Coastal
Stormwater, Non-Discharge and Water Supply Watershed regulations. This approval will expire when the
accompanying 404 or CAMA permit expires unless otherwise specified in the General Certification.
This approval is only valid for the purpose and design that you described in your application except as
modified below. If you change your project, you must notify us and you may be required to send us a new
application for a new certification. If the property is sold, the new owner must be given a copy of this
Certification and approval letter and is thereby responsible for complying with all conditions. If total wetland
fills for this project (now or in the future) exceed one acre, compensatory mitigation may be required as
described its 15A NCAC 2H .0506 (h) (6) and (7). For this approval to be valid, you must follow the
conditions listed in the attached certification.
If you do not accept any of the conditions of this certification, you may ask for an adjudicatory hearing.
You must act within 60 days of the date that you receive this letter. To ask for a hearing, send a written
petition conforming to Chapter 150B of the North Carolina General Statutes to the Office of Administrative
Hearings, P.O. Box 27447, Raleigh, N.C. 276 1 1-7447. This certification and its conditions are final and
binding unless you ask for a hearing.
This letter completes the review of the Division of Water Quality under Section 401 of the Clean Water

SUBJECT: Brunswick County, Widen NC 904 from US 17 at Grissettown
SR 1321; TIP. No. R-3313.
The North Carolina Department of Transportation proposes to widen NC 904 from US 17
at Grissettown to SR 1321 north of Longwood, and SR 1321 from NC 904 to NC 130.
This project is a Division Design and Construct project. The roadway is currently two
lanes with 18 feet of pavement and 8-10 feet of grassy shoulders. This facility will be
upgraded to a 2-lane shoulder section with 24-foot pavement width. The existing
shoulders are wide enough to accommodate the widening within the ditchlines without
disturbing the ditches. The project crosses jurisdictional surface waters at three locations.
However, only one crossing will require construction in surface waters. The culvert on
Cawcaw Swamp will be extended approximately 15 feet in each direction. The bridge
over Scippeo Swamp has recently been replaced and the bridge over Wet Ash Swamp is
scheduled for replacement in 2002. No additional bridge or culvert construction will be
necessary at these locations. This project will impact less than 0.1 acre of jurisdictional
waters of the United States through the culvert extension on Cawcaw Swamp. No
jurisdictional wetlands will be impacted.
Through telephone conversations with Mr. Jim Gregson of the Division of Coastal
Management (DCM) it has been determined that the project meets the conditions of
DCM's General Permit for activities in Estuarine Waters, Estuarine Shorelines, Public
Trust Areas and Coastal Wetlands. The NCDOT hereby requests issuance of a CAMA
General Permit for this project. Enclosed please find a project site map and a completed
application form. A check for th,?AMA processing fee, in the amount of $50.00, has
been transmitted to the DC V1,6 ' e in Raleigh. A copy of the check is also enclosed. It is
anticipated that construction f this pr ject will be authorized under a Department of the
Army Nationwide Permit N . 14. By copy of this letter, we are also requesting a
Department of the Army Na 'onwide Permit No. 14 and a 401 General Water Quality
Certification # 3103 (for NWP 14) from the N. C. Division of Water Quality. If you have
